Primary Purpose of Position

The RN Trainer serves a dual role as an Access Registered Nurse delivering direct patient care across NSCH sites (60%) and as a clinical education and onboarding lead for the nursing team (40%). This position supports the sustainability of the Director of Nursing and Nurse Manager roles and the resilience of the RN Workforce by building out a durable RN onboarding structure, escalation pathway, and standardized workflows/SOPs, while remaining an active clinical presence that keeps training grounded in real site conditions.

Requirements

  

Essential Functions

Clinical Practice — Access RN (~60%)

  • Provides direct and indirect nursing care across Salem, Peabody, and Gloucester sites as assigned, including RN visits, medication refill management, and patient triage/assessment (telehealth and/or in-person). 
  • Follows standing orders to complete nurse visits for chronic disease management, including but not limited to medication for opioid use disorder treatment, hypertension, diabetes
  • Administers immunizations (IM, sub-Q) and medications (oral, inhaled, nasal, IM, sub-Q;      performs procedures including EKGs, suture/staple removals, wound care, ear irrigations, PPD placement/read, and vital sign assessment
  • Provides health education to patients and families to support self-management and self-empowerment in maintaining optimal health
  • Completes patient forms within RN scope of practice, including prior authorization forms
  • Coordinates with      care team members outside the health center (specialists, visiting nurses)      to support continuity of care
  • Covers Epic in-basket management across sites as needed to support continuity during      staffing gaps
  • Provides triage coverage in partnership with site triage nurse teams
  • Collaborates and communicates with primary care providers and care team members — including huddle participation — to ensure quality, continuity of care, and a smooth clinical workday
  • Maintains accurate, timely documentation consistent with NSCH nursing standards
  • Helps provide RN access at whichever site has the greatest need — creating the cross-site      visibility and lived experience as an RN needed to standardize training and workflows, and allowing for mentorship relationships and elbow support to happen organically

Training, Onboarding & Clinical Skills Mastery (~40%)

  • Designs, implements, and continuously improves a standardized new hire RN onboarding program (oversight of RN visit ramp-up, mentorship pairing,  Epic RN workflow training including RN TH and on-site visits, RN skills competency checklists)
  • In collaboration with the DON and Director of Clinical Training, develops a clinical training program for continued education for the RNs based on the needs and deficiencies. 
  • Develops a needs assessment survey for nurses to assess educational and training needs. 
  • Serves as a first-line clinical resource/mentor for newly onboarded and existing RNs,      including real-time shadowing and precepting
  • Develops Student RN Internship infrastructure in collaboration with Nursing Leadership
  • Develops and maintains a clear escalation chain/support structure for RNs facing clinical or workflow questions
  • Collaborates with the DON, site Medical Directors, and CMO to develop, review, and      implement nursing workflows, SOPs, and standing orders (e.g., medication refill protocols, triage protocols, immunization administration)
  • Identifies workflow gaps or inconsistencies across sites through direct clinical access      experience and proposes standardized solutions
  • Uses data trends from clinical practice to drive self-directed learning, performance      improvement, and updates to training content
  • Tracks RN competency completion and documents training outcomes
  • Ensures RNs complete all trainings required for compliance reasons such as DPH and FTCA
  • Participates in nursing huddles/meetings across sites to reinforce training consistency and surface emerging issues
  • Supports the DON in nursing productivity and staffing initiatives by translating      training/workflow needs into actionable plans

Requirements

  • Current RN license in the Commonwealth of Massachusetts
  • Current CPR certification
  • Minimum 5 years of RN experience in an ambulatory/primary care or FQHC setting 
  • Demonstrated ability to train, mentor, or precept other nurses
  • Strong working knowledge of EHR systems (Epic) including in-basket management
  • Excellent communication, organizational, and critical thinking skills
  • Ability to travel between Salem, Peabody, and Gloucester sites; some evening/Saturday      coverage may be required
  • Sensitivity/skill working with complex patient populations (SUD, homelessness, serious      mental illness, etc.)
  • Bilingual Spanish/Portuguese preferred
