Responsibilities
Assesses, plans, implements, monitors and evaluates options and services to effect an appropriate individualized plan of care for patients across the acute care continuum using independent judgment and discretion.
Requirement
NYS RN License
B.S. in Nursing 3-5 yrs.
recent med/surg or cardiac experience,
PRI and/or case management certification preferred.
