Riam Recruiting is the exclusive search partner for a newly formed healthcare organization preparing to launch a Program of All-Inclusive Care for the Elderly (PACE) in January.

Help Launch a New PACE Program

This separate business entity focuses on helping older adults with complex medical and social needs remain safe, independent, and connected to their communities.

The organization is seeking an experienced Clinical Care Manager (RN) to serve as the primary nursing leader for its new PACE center. This is a unique opportunity to join before the first participant is enrolled and help build the clinical operation from the ground up.

Working closely with the PACE Director, Chief Officer, and other program leaders, the Clinical Care Manager will establish clinical workflows, prepare the center for launch, develop the nursing team, and shape a strong culture of coordinated, participant-centered care.

This is a hands-on leadership position. In addition to overseeing day-to-day clinical operations, the Clinical Care Manager will maintain a small participant panel and remain actively involved in assessment, care planning, care coordination, and direct nursing support when needed.

What You Will Do

Clinical Leadership and Program Launch

         Partner with executive and PACE leadership to prepare the clinical program for its January launch.

         Help develop and implement nursing workflows, policies, staffing plans, documentation standards, and clinical procedures.

         Oversee day-to-day clinical operations and promote safe, timely, and high-quality participant care.

         Recruit, supervise, mentor, and evaluate nursing and clinical team members, including RNs, LPNs/LVNs, CNAs, and medical assistants.

         Manage clinical staffing, scheduling, training, competency, and performance.

         Provide guidance during participant emergencies and complex clinical situations.

         Participate as an active member of the PACE leadership team and ensure the nursing perspective is represented in operational and strategic decisions.

Participant Assessment and Care Planning

         Complete initial, semiannual, annual, and change-in-condition nursing assessments in accordance with PACE requirements.

         Participate in the interdisciplinary team (IDT) and collaborate on individualized plans of care.

         Ensure care-plan interventions are implemented, monitored, documented, and adjusted as participants' needs change.

         Maintain a small participant panel and provide longitudinal clinical oversight, triage, education, and care coordination.

Geriatric Care and Care Transitions

         Support frail and medically complex older adults with chronic conditions such as diabetes, CHF, COPD, and other age-related needs.

         Coordinate care across the PACE center, participants' homes, hospitals, skilled nursing facilities, specialty offices, and contracted providers.

         Help manage transitions to and from hospitals, emergency departments, skilled nursing facilities, and home.

         Collaborate with hospital and SNF care teams on discharge planning, post-discharge follow-up, medication reconciliation, and continuity of care.

         Educate participants and caregivers regarding medications, symptom monitoring, self-management, and the participant's care plan.

Quality, Compliance, and Direct Care

         Maintain accurate, timely, and complete documentation in the electronic health record.

         Support compliance and audit readiness related to CMS, California PACE requirements, OSHA, CDC, infection prevention, and organizational standards.

         Participate in quality-improvement, infection-control, risk-management, and participant-safety initiatives.

         Provide direct nursing care within the RN scope of practice when needed, including triage, wound care, injections, specimen collection, and other approved clinical interventions.

         Participate in required leadership, IDT, and nursing on-call rotations.

Required Qualifications

         Graduate of an accredited school of nursing.

         Active California Registered Nurse license in good standing, or the ability to obtain one before starting.

         Current CPR/BLS certification, or the ability to obtain it within the required timeframe.

         At least two years of experience caring for a frail or older adult population, or the ability to complete required PACE training before working independently.

         Prior nursing leadership, clinical supervision, or management experience.

         Bilingual proficiency is required, with Cantonese or Mandarin strongly preferred. Candidates fluent in another Asian language, including Vietnamese or another Chinese dialect, will also be considered.

         Strong clinical assessment, triage, care-planning, and cross-setting care-coordination skills.

         Ability to work effectively with an interdisciplinary team and help build new processes in a start-up environment.

         Ability to complete local travel to participant homes, hospitals, skilled nursing facilities, and other community settings as needed.

Preferred Background

         Three or more years of RN experience working with medically complex older adults or in community-based geriatric care.

         Previous experience as a Director of Nursing, Assistant Director of Nursing, Nurse Manager, Clinical Manager, or similar nursing leader.

         Skilled nursing facility experience, particularly experience serving geriatric populations.

         PACE, home health, case management, managed care, Medicare Advantage, or managed Medicaid experience.

         Wound-care or case-management certification.

Ideal Candidate

The strongest candidate will combine credible RN leadership experience with a genuine commitment to serving older adults. They will be comfortable building a new clinical operation, collaborating across disciplines, creating structure where none currently exists, and remaining close to participant care.

This opportunity may be especially attractive to a current or former SNF Director of Nursing or clinical leader who wants to help older adults remain in their homes and communities through an integrated, relationship-centered care model.

Compensation and Benefits

         Base salary of $140,000-$149,000 based on experience and qualifications.
