Home Health Registered Nurse (RN)
Alpha Home Health & Hospice of King County is growing and seeking compassionate, skilled Registered Nurses (RNs) to join our Home Health team. This is a great position for entry to the industry or for new/newer RNs.

What You'll Do
As a Home Health RN Case Manager, you will provide skilled nursing care to patients in their homes while coordinating and managing their overall plan of care.
Key Responsibilities
- Perform comprehensive patient assessments and ongoing nursing evaluations
- Develop, implement, and update individualized care plans
- Provide skilled nursing services, including medication management, injections, wound care, and treatments
- Monitor patient progress and communicate changes in condition to physicians and care team members
- Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Complete accurate and timely EMR documentation
- Ensure compliance with Medicare, Medicaid, state, and agency regulations
- Participate in interdisciplinary team meetings and case conferences
- Promote patient safety, independence, and quality of life
Qualifications
- Active Washington State RN License
- Current BLS Certification
- Valid driver's license and insured vehicle
- Home Health or Hospice experience preferred, but not required
- Strong clinical, communication, and organizational skills
