Site: Mass General Brigham Home Care, Inc.

Mass General Brigham relies on a wide range of professionals, including doctors, nurses, business people, tech experts, researchers, and systems analysts to advance our mission. As a not-for-profit, we support patient care, research, teaching, and community service, striving to provide exceptional care. We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.

Job Summary

At Mass General Brigham, we believe exceptional care extends beyond hospital walls. Our Home Care team gives you the opportunity to step away from task-heavy, fast-paced settings and deliver thoughtful, one-on-one care—where you can truly see the impact of your work.

If you’re looking for more independence, stronger patient connections, and better work-life balance, this is a role where your work can feel different—in the best way.

We're offering a generous sign-on bonus of $5,000 (pro-rated for part-time) to welcome new hires to our team. Recognizing and valuing exceptional talent, we're eager to discuss the exciting details of this bonus during our interview process.

You’ll work with a diverse patient population and provide care where patients are most comfortable, at home.

We are recruiting for opportunities throughout Boston and surrounding communities, including:
• Charlestown, Cambridge, Boston
• Dorchester, Mission Hill, Roxbury, Jamaica Plain
• Hudson, Marlborough, Wayland, Sudbury
• Hudson, Marlborough, Wayland, Sudbury
• Newton
• Winthrop, East Boston, Chelsea, Revere
• Marblehead and Swampscott

Why you’ll like this team
You have independence, but you’re never on your own.
Flexible scheduling: Choose from full-time, part-time, Monday–Friday with no evenings, or weekends-only options. Complete 6–7 visits per day with the autonomy to manage your schedule and appointments.
Geography: Work within a focused service area designed to minimize travel between visits, giving you more time for patient care. Depending on the location, patients may be closer together, and in some cases, you may be able to walk between visits. Enjoy convenient access to the office when you need to replenish supplies. City parking passes are provided for new team members where applicable.
Dedicated Ancillary Support: Work closely with scheduling coordinators and support teams to help coordinate patient visits, maintain consistent care schedules, communicate schedule changes, and ensure you have the information and resources needed to deliver quality care in the home.
Strong team culture: Work alongside a collaborative team that values communication, teamwork, and supporting one another. Weekly interdisciplinary pod meetings, monthly team meetings, and quarterly in-person team days help keep the team connected.
Support to grow: Ongoing training, education, and opportunities to collaborate across disciplines.

A strong start matters
Home care can be a big transition, especially if you’re coming from a hospital or other clinical setting. Our onboarding is designed to help you feel prepared and confident working independently in the community. You’ll also have ongoing opportunities for training, education, and professional development.

Your time matters, too
Your schedule includes dedicated time for documentation, chart review, and care coordination. Our MGB electronic documentation system gives you easy access to doctors’ notes and orders, helping make communication and care coordination more seamless.

Your safety matters
We provide safety resources to support you in the field, including:
•GPS-enabled safety tools
•Real-time alert features
•Security-supported key fob access

What you’ll do
As an LPN, you’ll provide compassionate, skilled nursing care to patients in their homes while collaborating with the RN and interdisciplinary care team to support each patient’s individual needs.
•Provide nursing care and monitor patients’ conditions, reporting changes to the RN or provider as appropriate
•Assist with patient assessments, admissions, and care planning in collaboration with the RN
•Implement individualized plans of care and reinforce patient and caregiver education
•Administer medications and treatments as ordered and within the LPN scope of practice
•Coordinate care with patients, families, caregivers, and healthcare providers
•Maintain accurate clinical documentation and review patient charts to support quality care
