The Community Health Worker (CHW) Supervisor is responsible for the supervision, coordination, and day-to-day 

management of Community Health Workers (CHWs) who support patients, families and community members in 

addressing social determinants of health, navigating health and social service systems, coordinating care, supporting 

chronic disease self-management and promoting health and wellness. The CHW Supervisor provides leadership, coaching, 

training, and operational oversight to ensure high quality, patient-centered, community-based services aligned with 

BCHN’s mission and program goals. The CHW Supervisor collaborates with clinical teams, care management staff, 

behavioral health providers, community organizations, and external partners to improve patient engagement, care 

coordination, health equity, and access to essential resources.

Requirements

  • Supervise, coach, and evaluate Community Health Workers and related support staff. 
  • Conduct regular staff meetings, individual supervision sessions, and performance evaluations; provide timely feedback and 
  • support performance improvement as needed. 
  • Develop staffing schedules, assign caseloads and outreach responsibilities, and ensure appropriate coverage and workload 
  • distribution. 
  • Monitor CHW productivity, caseloads, service quality, documentation, and program performance metrics; use data to 
  • identify opportunities for improvement. 
  • Assess CHW training needs and coordinate ongoing training, professional development, and competency-building 
  • opportunities for CHW staff. 
  • Promote a culture of teamwork, accountability, patient centered care, and continuous quality improvement. 
  • Oversee outreach and patient engagement activities designed to connect patients and community members to primary care, 
  • preventive services, and community resources. 
  • Ensure CHWs identify and address barriers to care and social needs, including housing, food insecurity, transportation, 
  • benefits, employment, education, and other health-related social needs. 
  • Develop and maintain effective professional relationships with community-based organizations, social service agencies, 
  • schools, faith-based organizations, public health entities, and other community partners. 
  • Maintain knowledge of available community resources and strengthen referral pathways for patients and families, 
  • including closed-loop referral processes where applicable. 
  • Represent the organization at community meetings, events, and collaborative initiatives. 
  • Support and oversee health education, chronic disease self-management, and community engagement activities within the 
  • CHW scope of practice. 
  • Ensure timely, accurate, and complete documentation of patient encounters, screenings, referrals, interventions and 
  • outcomes in the electronic health record (EHR) and other designated systems. 
  • Prepare and review reports related to CHW productivity, program performance, outreach activities, social needs 
  • interventions, referrals, and outcomes. 
  • Track and monitor referrals, resource utilization, patient engagement, and outcomes; identify gaps and follow up on unresolved referrals as appropriate.
  •  Ensure CHW documentation and workflows comply with organizational policies, program requirements, and applicable regulatory and payer standards.
  • Maintain confidentiality and ensure compliance with HIPAA and organizational privacy and security requirements.
  • Support compliance with applicable HRSA Health Center Program requirements, FQHC requirements, NYS regulations, grant requirements, and other federal, state, and local requirements relevant to CHW programs.
  • Support data collection, validation, and reporting for the Uniform Data System (UDS), quality improvement initiatives, value-based care programs, and grant-funded programs, as applicable.
  • Assist with audit readiness, site visits, program reviews, and performance improvement activities related to community health and population health programs.
  • Maintain knowledge of and compliance with applicable FQHC, HRSA, Article 28, HIPAA, OSHA, infection control, patient privacy, and federal, state, and local regulatory requirements.
  • Supports audit readiness, survey preparation, documentation standards, and organizational policies as applicable to the role.
  • Perform other duties, as assigned based on organizational needs, including support for new site openings, mobile health operations, emergency preparedness, community outreach and special projects.

Qualifications

  • Bachelor’s degree in health education, Public Health, Community Health, Health Education and Promotion, Nursing, Social Work, or related field.
  • Minimum of three (3) years of experience in community health, care coordination, population health, case management, social services, patient navigation, or related field.
  • Minimum of two (2) years of supervisory or team leadership experience, preferably supervising CHWs or other community-based staff.
  • Experience working with medically underserved, vulnerable, and diverse populations.
  • Experience in a Federally Qualified Health Center (FQHC), healthcare organization, community-based organization, or similar setting preferred. 

 

Knowledge, Skills, and Abilities

  •  Knowledge of social determinants of health, community resource systems, and referral/navigation processes.
  •  Understanding of population health, care coordination, chronic disease self-management, health education, patient engagement and community outreach principles.
  • Demonstrated leadership, coaching, staff development, and team building skills.
  • Excellent communication, interpersonal, organizational problem-solving, and conflict resolution skills.
  • Ability to develop and maintain effective professional partnerships with clinical teams, community organizations, and other stakeholders.
  • Proficiency with electronic health records, data reporting systems, and standard office technology; ability to monitor and interpret program performance data.
  • Knowledge of value-based care, care management, population health initiatives, NYS Social Care Network and CHW related reimbursement models preferred.   

Salary Range: The base salary range for this position is $70,000 – $80,000 annually.


The total compensation for the finalist selected for this role will be determined based on various factors, including but not limited to scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The base salary range listed above is a good faith determination of potential base compensation at the time of this job advertisement and may be modified in the future.
