Nurse Practitioner / Physician Assistant – Inpatient Palliative Care
Location : New Jersey
Position Overview
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join a well-established inpatient palliative care team. This is an excellent opportunity for a compassionate provider who is passionate about delivering high-quality, patient-centered care to individuals living with serious illness.
You will work alongside an experienced interdisciplinary team, providing consultative and follow-up palliative care services in a collaborative hospital environment.

Responsibilities
-
Provide inpatient palliative care consultations and follow-up visits.

Assess and manage complex symptoms for patients with serious or advanced illnesses.

Lead goals-of-care discussions and advance care planning with patients and their families.

Collaborate with physicians, nurses, social workers, care coordinators, and other healthcare professionals.

Coordinate patient transitions, discharge planning, and continuity of care.

Maintain accurate and timely clinical documentation.

Deliver evidence-based, compassionate, and patient-centered care.
Qualifications
-
Active New Jersey Nurse Practitioner or Physician Assistant license.

National board certification.
-
DEA and CDS registration, or ability to obtain before start date.

Minimum of 2 years of inpatient palliative care experience required.

Palliative care certification (ACHPN, CHPN, or equivalent) preferred.

Excellent communication, clinical judgment, and interpersonal skills.

Ability to work effectively within an interdisciplinary team.
