Cancer Care Associates of York is seeking a full-time Oral Oncology Nurse Navigator to join our dedicated, compassionate team!
What you will be doing...
JOB SUMMARY: The oral oncology nurse navigator provides coordination for patients on oral oncolytic aspects of evaluation, treatment, planning, education, and connection with other support services. In addition, assists in clinical education to the nursing department and medically integrated dispensary.
Essential Functions- Job Duties:
1) Assessment:
a. Assesses oral therapy patient needs and identify barriers to oncology care at first encounter, collaborate to make appropriate referrals and reassess throughout their continuum of care.
b. Provides direct contact with patients, families or caregivers to assess the patient’s needs in symptom management and toxicity assessment using the nursing process.
c. Contacts all patients with new start of oral oncolytic therapy for adherence check and initiation of side effect management within one week of start.
d. Assists in triaging toxicities related to oral chemotherapy prescriptions.
e. Monitors oral therapy patients during hospital admissions and coordinates care upon discharge.
f. Assesses noncompliance concerns- adherence to medications – identify issues
2) Diagnosis (problem, risk, need for health promotion, syndrome):
a. Identifies patient problems, at risk issues, need for education, potential toxicities.
b. Identifies and reports adverse events or non-compliance related to oral therapy to providers.
3) Planning (expected outcomes):
a. Collaborates with Nurse Manager in regard to clinical oversite of patients.
b. Collaborates with APP regarding high-risk oral oncology education visits.
c. Provides consistent communication with providers regarding plan of treatment and all necessary follow-up care including documentation of adherence to oral oncolytic and support medications.
d. Collaborates with nursing team on nurse visits/labs/triage phone calls for patients on oral only or IV/oral dual regimens.
e. Coordinates with Oral Oncology MA re: plan of care for patients
f. Collaborate with dispensary team - assist with any clinical questions, verify oral medications for dispensary final check before dispensing to patient, communicate on drug access/authorizations.
4) Implementation / rationale:
a. Assists in educating nursing staff, patients and caregivers regarding new treatments, clinical programs, and systems necessary to support changing patient needs and adherence in order to promote safe and effective treatments.
b. Assists the providers in education of patients regarding the disease process, treatment goals and directs necessary referrals for comprehensive care.
c. Collaborates with other navigators outside the organization to share best practices and increase patient resources.
d. Provides comprehensive documentation of adherence and tolerance to oral oncolytic and support medications.
5) Evaluation:
a. Participates in nursing meetings, medically integrated dispensary and APP meetings to provide coordination of care across all disciplines as needed.
b. Evaluates patient outcomes, tolerance of therapy and ongoing adherence for new therapy start and high-risk patients.
6) Maintains regular and punctual attendance.
7) Complies with all practice policies and procedures including CCAY’s Code of Conduct.
8) Practices ethically and in a manner that is congruent with cultural diversity and inclusion principles.

What makes you qualified?
- Nursing diploma, Associate’s Degree required or higher preferred
- Current licensure as a Registered Professional Nurse is required, issued by the PA State Board of Nursing.
- Certificate in Oncology Nursing, issued by the Oncology Nursing Certification Corporation, preferred (obtain within 6-12 months of hire).
- CPR certification
- At least 3 years of RN experience, with 1-2 years of oncology experience required.
