The Respiratory Therapist (RT) Pulmonary Care Coordinator serves as a clinical resource and patient advocate for individuals with chronic pulmonary conditions, with a primary focus on Chronic Obstructive Pulmonary Disease (COPD) management and Inspire therapy coordination. This role is responsible for facilitating comprehensive patient care, improving clinical outcomes, reducing hospital readmissions, coordinating multidisciplinary services, and ensuring patients receive timely education, follow-up, and support throughout their treatment journey.

Required: Associates Degree, Registered Respiratory Therapist, Respiratory Care Practitioner, BLS or CPR within 90 days of hire
Pay: Based on experience, starting at $29.08
Responsibilities
Assist in managing COPD registries, reports, and population health initiatives., Collaborate with physicians, advanced practice providers, nursing staff, case management, and other healthcare team members to develop individualized care plans., Conduct telephone follow-up and patient assessments to identify barriers to care and address clinical concerns., Coordinate patient referrals, consultations, diagnostic testing, and procedural scheduling., Coordinate post-hospitalization follow-up appointments and interventions to reduce readmissions., Educate patients on Inspire therapy, eligibility requirements, treatment expectations, and follow-up care., Ensure patients complete required testing and appointments within established timeframes., Facilitate communication between pulmonology, sleep medicine, ENT providers, surgery scheduling teams, and device representatives., Facilitate referrals to pulmonary rehabilitation, smoking cessation programs, home health services, and community resources., Identify and manage high-risk COPD patients through proactive outreach and ongoing monitoring., Inspire Therapy Coordination., Maintain accurate documentation and monitor program outcomes., Monitor COPD quality metrics and assist with performance improvement initiatives., Provide patient education regarding COPD disease management, inhaler techniques, oxygen therapy, smoking cessation, pulmonary rehabilitation, and symptom recognition., Serve as the primary coordinator for patients undergoing evaluation and treatment with Inspire therapy for obstructive sleep apnea., Track patient progress through all phases of the Inspire program, including evaluation, implantation, activation, and ongoing follow-up.Requirements
