Licensed Practical Nurse (LPN) — Emerald Cares
Location: Fargo, ND
Employment Type: Full-time, Part-time, or PRN
Schedule: 12-hour shifts, 6:00 a.m.–6:00 p.m. or 6:00 p.m.–6:00 a.m., based on staffing needs
Pay $32/hr
Reports To: Registered Nurse or Nursing Supervisor
Position Summary
The Licensed Practical Nurse provides person-centered nursing care to individuals with developmental disabilities and complex medical needs in Emerald Cares’ residential and community settings. Under the direction of a registered nurse or authorized healthcare provider, the LPN delivers prescribed care, monitors health conditions, and supports each individual’s dignity, choices, and independence.
Essential Responsibilities
- Collect health information, monitor vital signs, and promptly report changes in condition to the supervising RN or healthcare provider.
- Contribute to individualized nursing care plans and implement assigned interventions.
- Administer medications and treatments according to provider orders, agency policy, and authorized scope of practice.
- Provide specialized care within demonstrated competency and required supervision, including tracheostomy and ventilator care, enteral feeding and tube care, wound care, and seizure monitoring.
- Monitor responses to medications and treatments and report adverse reactions or concerns.
- Coordinate prescription refills, medical supplies, and clarification of orders with the appropriate RN, provider, or pharmacy.
- Recognize urgent health concerns, initiate appropriate actions within training and scope, and contact emergency services when needed.
- Collaborate with RNs, healthcare providers, therapists, case managers, families, and guardians to support continuity of care.
- Assist with appointments, hospital transitions, and follow-up care; accompany individuals on community outings when nursing support is required.
- Provide guidance to CNAs and direct support professionals regarding assigned care activities, consistent with the RN’s care plan and agency policy.
- Reinforce health education provided by the RN or healthcare provider.
- Complete accurate, timely documentation, including medication and treatment records, nursing notes, incident reports, and shift handoffs.
- Follow infection prevention practices and maintain a safe care environment.
- Assist with personal care, repositioning, transfers, and daily activities as needed.
- Participate in staff training, care conferences, quality improvement activities, and required meetings.
- Protect confidentiality and uphold individual rights, informed choices, and respectful care.
- Report suspected abuse, neglect, exploitation, medication errors, and other safety concerns according to agency procedures and applicable requirements.
Required Qualifications
- Graduate of an approved practical nursing program.
- Current LPN license authorizing practice in the state where services are provided.
- Current BLS/CPR certification.
- Strong observation, communication, documentation, and organizational skills.
- Ability to prioritize assigned care and recognize when RN or provider guidance is needed.
- Completion of required background screening, orientation, and competency validation.
Preferred Qualifications
- Experience in home care, residential services, long-term care, or care of individuals with developmental disabilities.
- Experience caring for individuals with ventilators, tracheostomies, feeding tubes, seizure disorders, or other complex medical conditions.
- Familiarity with electronic documentation systems, including Therap.
Physical and Work Requirements
- Ability to perform essential nursing and direct care duties, with reasonable accommodation as appropriate.
- Ability to assist with repositioning and transfers using prescribed equipment and staff assistance.
- Ability to work scheduled shifts, including weekends and holidays as assigned.
- Potential exposure to infectious materials and other healthcare-related hazards; required protective equipment and safety procedures must be followed.
