POSITION SPECIFICS

Title:  Advance Practice Nurse Prescriber – Wound Care

FTE: .5FTE  (20 hours/week)

Schedule: Monday and Thursday from 8:00am – 6:00pm  

Holiday Rotation: None

Weekend Rotation: None

On Call Requirements: None

POSITION SUMMARY

The Advance Practice Nurse Prescriber (APNP) – Wound Healing will provide patient care services for wound care patients throughout the continuum of care.  This position will maintain expertise in assessment, diagnosis, and treatment while focusing these skills towards wound care management and supportive care.  This position will collaborate with all members of the multi-disciplinary healthcare team (MDHCT), including specific collaboration with physicians, other advanced practice providers, and members of the Healogics team in order to meet the individual patient needs. 

POSITION TECHNICAL RESPONSIBILITIES

  • Assess and diagnose patient wounds and develop and revise treatment plans in accordance with Healogics’ protocols to meet patient needs

  • Evaluate interventions in relationship to desired outcomes, including quality improvement and care coordination

  • Order and interpret diagnostic studies.

  • Perform treatment procedures that can be performed within a clinic setting or outside of the operating room

  • Correlate risk factors associated with wound conditions.

  • Assess response to pharmacologic and non-pharmacologic therapies.

  • Select diagnostic studies to evaluate wound conditions.

  • Collaborate with other members of the MDHCT to formulate teaching plans for wound care treatment modalities for the patient, and / or their significant others

  • Order & coordinate care with other services/specialties.

  • Medical record and billing documentation in compliance with SPH and Healogics standards

  • Educate and counsel patient and/or family care giver regarding diagnosis, treatment and prevention.

  • Promote wellness through screening and prevention.

  • Order, prescribe and administer medications, specialty dressings, and / or devices as allowed under Wisconsin law. 

  • Participate in Quality Performance initiatives.

  • Assess, plan, order, and provide oversight for Hyperbaric Oxygen Therapy

  • Engage in continuous learning to ensure the practice of evidence-based medicine within specialty area.

POSITION REQUIREMENTS

Education

Required: Masters of Science degree in nursing or equivalent.

Preferred: None

Experience

Required: 3-5 years in wound, vascular, or complex patient care management at an RN level

Preferred: 1-3 years clinical experience at an advanced practice level for wound, vascular, or complex patient care management

Licenses and Registrations

Required: Valid State of Wisconsin RN and Advanced Practice Nurse Prescriber (APNP) license.

DEA registration.

Preferred: None

Certification(s)

Required: Basic Life Support (BLS) required within 3 months of appointment.

Preferred: Advanced Cardiac Life Support (ACLS)
