Description
Care Coordination & Oversight
o Develop, update, and monitor individualized resident care plans in collaboration with the
interdisciplinary team.
o Ensure that care plans reflect the physical, emotional, and psychosocial needs of each resident.
o Conduct routine assessments and communicate changes in condition to the physician and
appropriate staff.
• Resident Care & Advocacy
o Monitor resident health status and provide direct nursing care as needed.
o Advocate for residents’ rights and quality of life, ensuring dignity, respect, and safety.
o Act as the primary point of contact for families regarding resident care updates.
• Team Leadership & Supervision
o Supervise and support CNAs and other nursing staff in the delivery of daily care.
o Provide training, guidance, and feedback to staff to ensure adherence to care standards and
policies.
o Assist in performance evaluations and corrective actions when necessary.
• Documentation & Compliance
o Accurately document evaluations, treatments, and care interventions in the medical record.
o Maintain compliance with state, federal, and facility regulations, including infection control and
safety protocols.
o Prepare for and participate in state surveys, audits, and quality assurance activities.
o Complete MDS information on residents as required.
• Communication & Collaboration
o Coordinate with physicians, therapists, and other healthcare professionals to ensure continuity of
care.
o Facilitate care conferences with residents and families.
o Report significant changes, incidents, or concerns promptly to the Director of Nursing.
