Hourly Wage: $37.28

About the Role:
Provides public health nursing services to individuals, families, and population groups. Nursing actions are directed toward the goals of prevention, assessment, risk reduction and health status improvement for individuals, families, and communities. Activities include, but are not limited to, systematic analysis of health data, care plan development, health education and advocacy, coalition building, and collaborating with community partners to promote the health of a population. Often collaborates and consults within multi-disciplinary and multi-agency teams to achieve desirable outcomes for families and communities. Serves as the clinical team leader for all clinic staff. 
  • Assists Clinical Services Manager in directing and assigning work for clinic staff, recommending improvements in workflow, coordinating clinics, and working with Clinical Services Manager to ensure operational goals of the Department and Clinical Division are met and services provided are safe, healthy, evidenced-based and appropriate.
  • Performs comprehensive individual and family assessments that include physical, psycho-social, developmental, family functioning, substance abuse and violence issues, assessment of basic needs, resources and supports and access to health care.
  • Provides care coordination, referral and follow-up to individuals who are members of high-risk or vulnerable populations.
  • Uses public health surveillance/disease investigation methods and performs disease surveillance and screening to curtail the incidence and prevalence of disease conditions that threaten the health of the community.  
  • Assists staff in the delivery of essential public health nursing services as appropriate or necessary.  
  • Contacts community providers as necessary to prevent the spread of disease and provide recommendations on treatment/follow-up and requirements. 
  • Assists medical providers (Medical Advisor and Advanced Registered Nurse Practitioner) with revisions to standing orders, protocols, and procedures for the delivery of service.  
  • Ensures procedures meet Federal, State, and local regulations.  
  • Works with and obtains approval of Medical Advisor and Advanced Registered Nurse Practitioner for medical protocols.
  • Conducts evaluations of program results/outcomes to measure effectiveness and adequacy of disease prevention/health promotion activities.  
  • Gathers epidemiological and statistical data to assist with disease prevention/health promotion intervention and programming. 
  • Acts as a resource and recommends/refers clients to appropriate resources.
  • Maintains knowledge of program area(s) and shares new developments. 
  • Assists with training others in departmental and divisional procedures/protocols. 
  • Compiles and prepares written and statistical reports, program records, documentation, and historical database information. 
  • Bachelor's degree in Nursing required.  
  • Three (3) to five (5) years of experience in public health nursing required. 
Monday - Friday 
8 a.m. - 4:30 p.m.
