Job Details:
- Currently licensed with the state of Texas.
- Excellent verbal and written communication skills.
- Experience with electronic medical records is strongly desired.
- Home Health or hospice assessments including consents in the home, hospital or facility.
- MINIMUM of 1 year of Hospice and Home health experience with verifiable references.
- Wellsky/Kinnser experience REQUIRED.
- Be available by phone during normal working hours, when on call, and when on back up call.
- Performs patient visits according to the Plan of Care or as patient need dictates.
- Deliver care and instruction that is appropriate to the patient’s plan of care.
- Coordination of care with wound care agencies, doctors and team members via Kinnser.
- Perform case management activities for assigned patients.
- Respond to verbal and written issues, concerns, or complaints from home care patients, families, physicians, nurse.
- Comprehensive Assessments of patients utilizing the OASIS data sets or Hospice HIS data set.
- May entail GIP care.
- Meet with the IDT/case conference team once every other week.
- Experience working with children highly considered.
- Coordinate schedule to make most use of time during the day.
- Will require driving outside of the immediate residential area to provide patient care.
- Concierge Nursing required.

COVID-19 considerations:
- Bachelor's (Preferred).
- Nursing: 1 year (Highly Preferred).
