A growing healthcare organization is seeking an experienced Nurse Practitioner to provide specialized care for patients experiencing acute and chronic pain conditions. This full-time opportunity offers a flexible schedule and the ability to work closely with interdisciplinary healthcare teams to improve patient function, comfort, and overall quality of life.
The ideal candidate is passionate about patient-centered care, experienced in pain management, and comfortable working with complex patient populations in post-acute care and long-term care settings.
Position Summary
The Nurse Practitioner will evaluate, diagnose, and manage patients with acute and chronic pain conditions while collaborating with healthcare providers, rehabilitation teams, and facility staff. This role focuses on improving patient outcomes through comprehensive pain management strategies, ongoing monitoring, and individualized treatment plans.
Key Responsibilities
- Conduct comprehensive patient evaluations and assessments
- Develop and implement individualized pain management treatment plans
- Monitor patient progress and adjust treatment approaches as appropriate
- Evaluate medication utilization and effectiveness
- Assess functional status, mobility, and rehabilitation progress
- Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams
- Educate patients, families, and caregivers regarding treatment plans and pain management strategies
- Maintain accurate and timely clinical documentation
- Support quality improvement initiatives and patient outcome tracking
- Ensure compliance with applicable clinical and regulatory standards
Qualifications
Required
- Master’s or Doctoral degree from an accredited Nurse Practitioner program
- Active Nurse Practitioner license or eligibility for licensure
- DEA registration or eligibility to obtain
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal abilities
Preferred
- 1–3 years of pain management experience
- Experience working with geriatric, rehabilitation, post-acute, or long-term care populations
- Experience managing chronic pain conditions and medication therapy
- Background in physical medicine, rehabilitation, or related specialties
