Job Description:

Duties/Responsibilities:
- Conduct psychosocial assessments of patients and families which will include identification of current stressors both related and unrelated to cancer which may impact patient and family coping and/or compliance.
- Minimize emotional distress through individual, family and group counseling.
- Employ cognitive-behavioral interventions to reduce stress, maximize coping skills and increase patient/family self-control.
- Work collaboratively with interdisciplinary oncology team to create comprehensive plan of care for patients.
- Provide education about the health care system, adaptation to physical, psychological and social changes imposed by illness and support medical information given by physicians and staff.
- Coordinate and facilitate appropriate referrals to community resources for: mental health, substance abuse, visiting nurse, Hospice, nursing home placement, medications, home IV therapy, DME and entitlement programs.
- Assist patient with problem solving related to family coping, employment, financial concerns, adaptation to the healthcare system, transportation and changes in patient’s functional level.
- Advocate for patients within a complex health system, identify and overcome barriers to recommended care and facilitate appropriate services.
- Design, implement and monitor innovative programs to facilitate optimum adjustment and function (i.e. patient support funds, transportation assistance programs, patient resource/enhancement boutique).
- Provide information to professional and lay groups within the community regarding the physchosocial impact of cancer.
- Improve and facilitate communication between patients, insurers and staff.
- Provide continuity of patient care, act as a liaison to CSI staff, and community providers, collaborate on oncology in-patient needs, assist in facilitating patient’s care in the most appropriate setting.
- Optimize patient and family functioning by arranging quality services in the most efficient and cost effective manner in complex situations.
- Identify need for resources, assess appropriateness and management of resources from both a Clinically sound and cost effective perspective.

Education/Experience Required:
- Masters Degree in Social Work
- Experience in individual, family and group counseling.
- Developed knowledge of community resources and payers/vendor restrictions.
- Experience with medically compromised clients.
Registration/Certification:
- LICSW.
- Certification in Advanced Social Work Case Management
Physical/Environmental Requirements:
This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
Pay Range:
$28.90 - $51.34This position is covered by a collective bargaining agreement. The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. The pay range for this position is established and governed by the terms of the applicable collective bargaining agreement. Employees hired into this position will receive compensation consistent with the terms of the applicable collective bargaining agreement.
