While looking for a FT RN to fill this role permanently we are open to hiring a Part Time, Contract or Temporary LPN or RN to help with our Adult Services Programs. 

Position
: Registered Nurse (RN) or LPN

Location: Laradon fosters a collaborative, team environment; working on-site in Denver, CO and/or traveling to clients' homes.

Supervision: Reports to Director of Adult Services

Schedule: Flexible

Starting Hourly Range: $70-$80 RN or $55-$65 LPN  based off education and experience

Job Summary                    

The Registered Nurse (RN) provides primary nursing support and medical coordination for Laradon’s Community Living Services (CLS) program, with additional nursing support provided to Adult Day, CYC, School, and other Laradon programs as needed. The RN is responsible for assessing and coordinating the healthcare needs of assigned individuals, supporting medication management, maintaining health care plans and medical protocols, coordinating medical appointments and follow-up care, maintaining accurate medical documentation, and supporting healthcare compliance requirements.
The RN works collaboratively with individuals served, families and guardians, CLS staff and leadership, case managers, contracted caregivers, physicians, pharmacies, hospitals, and other healthcare providers to promote continuity of care and ensure medical needs are appropriately addressed.
Key Duties & Responsibilities
Clinical Nursing & Medical Coordination
  • Monitor the health status, medications, medical needs, and health-related concerns of assigned CLS individuals.
  • Provide nursing assessments and appropriate clinical support, treatment, follow-up, and coordination of care as needed.
  • Coordinate medical appointments, annual physicals, screenings, laboratory work, hospital admissions/discharges, and recommended follow-up.
  • Communicate with individuals, families/guardians, contracted caregivers, CLS staff, providers, pharmacies, case managers, and other members of the interdisciplinary team regarding medical needs.
  • Serve as a resource to staff regarding medical concerns, health documentation, medication-related issues, and individual healthcare needs.
  • Provide or coordinate nursing support for specialized medical needs and procedures.
  • Provide nursing support to other Laradon programs, including Adult Day, CYC, and School, as assigned or needed.
  • Coordinate annual hearing and vision screenings as needed.
  • Support health and safety initiatives across programs.
CLS Home Visits & Residential Support
  • Complete monthly home visits for assigned CLS individuals to assess overall health and safety, review medical needs, identify changes in condition or care needs, and determine whether additional follow-up is needed.
  • Document each home visit, including observations, identified concerns, recommendations, and required follow-up.
  • Follow up on medical or health-related concerns identified during home visits and communicate significant concerns to appropriate CLS leadership, guardians, caregivers, providers, and other team members.
  • Provide medical information and consultation to Residential Program Coordinators and CLS leadership as needed.
  • Assist with identifying gaps in medical services, documentation, communication, and follow-up.
Medication Management & Health Care Plans
  • Maintain responsibility for nursing assessment, clinical judgment, medical decision-making, medication management, health care plans/medical protocols for assigned individuals.
  • Monitor medication administration and review Medication Administration Records (MARs) for accuracy and compliance.
  • Obtain, review, send, and track Physician Orders and other required medical documentation.
  • Configure and update MARs based on current Physician Orders.
  • Develop, implement, review, and update health care plans/medical protocols based on individual needs and required review timelines.
  • Provide information and instruction to staff regarding health care plans/medical protocols, medication-related requirements, and individual healthcare needs.
  • Identify medical concerns that require nursing assessment or intervention and communicate recommendations to appropriate staff and leadership.
  • Collaborate with administrative and support staff on medical documentation and tracking while retaining responsibility for nursing judgment and clinical decisions.
Medical Documentation, Records & Compliance
  • Obtain and monitor required medical documentation, including physicals, Physician Orders, medication orders, evaluations, screenings, and follow-up records.
  • Maintain accurate medical records, tracking spreadsheets, databases, electronic files, and medical monitoring tools.
  • Scan, index, file, transcribe, and process medical documentation accurately and timely.
  • Monitor designated medical email and ensure documentation is appropriately processed and maintained.
  • Maintain quarterly tracking of outstanding medical documentation, appointments, screenings, and follow-up needs.
  • Prepare documentation for audits, file reviews, surveys, and regulatory reviews.
  • Ensure medical records are maintained in accordance with Laradon policies, confidentiality requirements, nursing standards, and applicable regulatory requirements.
  • Monitor medical documentation and compliance indicators and communicate concerns to appropriate leadership.
  • Participate in interdisciplinary meetings, quality improvement initiatives, and medical-related special projects.
Communication & Coordination
  • Maintain effective communication with individuals served, families/guardians, contracted caregivers, medical providers, case managers, governing agencies, and Laradon staff.
  • Communicate with Spanish-speaking providers and other healthcare professionals as needed to facilitate medical coordination.
  • Coordinate with the Training Specialist regarding medical requirements and training for CLS contracted caregivers.
  • Serve as a clinical resource to staff and program leadership regarding individual medical needs and healthcare processes.
Training & Staff Education
  • Provide education and training to employees regarding health, medication, medical protocols, emergency procedures, health care plans, and individual-specific healthcare needs.
  • Assist with QMAP and QMAP Refresher training as assigned.
  • Coordinate with the Training Specialist to support completion of required medical-related training.
  • Maintain required professional and organizational training and development.
Administrative Responsibilities
  • Maintain regular and punctual attendance and remain present in office and program environments as required.
  • Complete miscellaneous administrative duties, special projects, and other responsibilities as assigned.
  • Maintain required professional and organizational training, including orientation, mandated reporting, incident reporting, First Aid/CPR, crisis intervention, and required on-the-job training.
Minimum Position Requirements
  • Current Colorado RN license in good standing.
  • Two or more years of nursing experience.
  • Experience with individuals with intellectual/developmental disabilities preferred.
  • Valid Colorado driver's license, good driving record, and reliable transportation.
  • Strong communication, organization, customer service, and interpersonal skills.
  • Ability to maintain confidentiality and manage multiple medical and regulatory responsibilities.
  • Ability to work collaboratively with individuals served, families, caregivers, medical providers, and program staff.
  • Ability to meet the physical requirements of the position, including walking, standing, sitting, bending, kneeling, crouching, and other physical activities as required.
  • Ability to communicate effectively and perform required job functions in program and residential environments.
