Manager, Care Management

Overview

The Manager, Care Management provides strategic, operational, and regulatory leadership to strengthen Quartz’s care management infrastructure. This role leads projects and cross‑functional initiatives that ensure regulatory compliance, advance evidence‑based care management practices, and support patient activation, community partnerships, and fiscal stewardship. The Manager serves as a subject matter expert to interdisciplinary teams and collaborates with senior leaders to drive tactical and strategic outcomes aligned with Quartz’s mission and values.

Responsibilities

• Develops project plans and leads initiatives that enhance the care management infrastructure.
• Monitors CMS, NCQA, and State regulations.
  • Provide guidance to CM Leadership team on the system changes needed; ensure timely dissemination of information to staff.
• Participates in state‑sponsored meetings as requested and communicates relevant updates to stakeholders.
• Ensures care management functions meet all regulatory and accreditation requirements.
• Provides project management for key initiatives and drives critical projects to completion.
• Identifies opportunities and manages collaboration with other Quartz departments and key stakeholders.
• Identifies areas for self and staff development and actively pursues resources to meet development objectives.
• Creates and delivers presentations, reports, and profiles to communicate successes, opportunities, and the importance of social determinants of health (SDoH) and community development.
• Participates in and/or conducts focus groups supporting growth and SDoH strategies, as needed.
• Develops materials for internal and external presentations and communications regarding Quartz’s business goals, market strategy, policy, positioning, and outcomes in the care management environment.
• Participates in financial forecasting and planning with the management team.
• Develops and presents comprehensive analyses to support financial and program recommendations.
• Evaluates opportunities for cost‑effective change and manages implementation of approved initiatives.

Qualifications

• Bachelor’s or Associate’s degree in health education, nursing, or a related health field.
• 5+ years of experience in a healthcare‑related field.
• At least one (1) year of professional supervisory/management people-leading experience
• At least three (3) years of experience in case management.
• Experience supporting regulatory compliance (CMS, NCQA, State) and interdisciplinary clinical teams.
• Accountability for team performance and results; continually expands departmental capabilities using sound judgment and interpretation.
• Demonstrated ability to develop and manage initiatives that deliver results while fostering teamwork and collaboration.
• Recognized content expert in care management across clinical, social, behavioral, and population health domains.
• Strong understanding of government and regulatory requirements.
• Excellent organizational skills with the ability to manage priorities and meet deadlines.
• Strong initiative, problem‑solving, critical thinking, and decision‑making skills.
• Excellent written and verbal communication skills; able to clearly convey ideas, instructions, and feedback to staff, members, and partners.
