Job Title: Certified Nursing Assistant (CNA) – Skilled Nursing / Long-Term Care
Job Summary: We are seeking a resilient, dedicated Certified Nursing Assistant (CNA) to deliver high-quality, patient-centered care in a fast-paced Skilled Nursing and Long-Term Care environment. Operating under the supervision of nursing leadership (Unit Manager / ADON / DON), you will carry out individualized plans of care, assist residents with daily living activities, maintain a safe clinical environment, and support overall unit workflow.
Key Responsibilities:
- Direct Resident Care: Deliver comprehensive basic nursing care—including personal hygiene, mobility, positioning, nutrition, and hydration—in accordance with each resident’s individualized care plan.
- Care Plan Execution & Monitoring: Assist in implementing and evaluating patient care plans, observing changes in physical or behavioral conditions, and reporting findings promptly to supervising nurses.
- Safety & Environment Management: Maintain a clean, safe, and organized care environment following infection control standards and safety guidelines.
- Team Collaboration: Work closely with nursing staff, assistants, and interdisciplinary team members to ensure smooth shift transitions and efficient patient care.
Qualifications & Requirements:
- Experience: Minimum 2+ years of CNA experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), or Medical-Surgical setting [Required].
- Licensure: Active State Certified Nursing Assistant (CNA) license/registration [Required].
- Certifications: Active Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross [Required].
- Vaccination: Up-to-date COVID-19 vaccination or approved exemption [Required].
- Core Competencies: Ability to thrive in a high-volume, fast-paced care setting, strong interpersonal skills, schedule flexibility, and a patient-first work ethic.
