High Focus Centers in Branchburg is a premier provider of outpatient treatment programs for mental health, substance use and co-occurring disorders. Our highly trained staff create individualized, comprehensive and robust treatment plans designed to meet each client’s treatment goals. At High Focus Centers in Branchburg we are currently seeking a Nurse Practitioner to join our amazing team of behavioral healthcare professionals providing Partial Hospitalization (PHP) and Intensive Outpatient (IOP) treatment services to our co-occurring adult and adolescent clients.
The Nurse Practitioner provides medical and/or psychiatric evaluation, assessment, medication management, care coordination, and ongoing treatment services for individuals receiving behavioral health and substance use disorder treatment services. This role collaborates with interdisciplinary treatment teams and internal and external healthcare providers to support high-quality, person-centered care in accordance with organizational, regulatory, and professional practice standards.

- Hourly Rate: $84.00/hour
Essential Duties and Responsibilities
- Complete history and physical assessments, evaluations, reassessments, and other clinical documentation as required.
- Review laboratory results, monitor blood levels, review physician orders, and support ongoing medical and medication management needs.
- Provide medication management services, including psychotropic medication management where applicable based on licensure and scope of practice.
- Diagnose and treat medical and/or psychiatric conditions in accordance with scope of practice and applicable regulations.
- Coordinate client care with internal and external healthcare providers, including primary care physicians, psychiatrists, specialists, pharmacies, and other treatment providers as appropriate.
- Support discharge planning and continuity of care efforts, including medication coordination and aftercare recommendations.
- Collaborate with nursing and clinical teams to support cost-effective medication management and completion of medication prior authorizations when needed.
- Provide education and clinical support to staff regarding medical, psychiatric, behavioral health, and medication-related concerns as appropriate.
- Participate in treatment team meetings, clinical review meetings, peer review processes, and departmental meetings as required.
- Maintain timely, accurate, and compliant documentation in accordance with CARF standards, licensing requirements, payer requirements, and organizational policies.
- Must exercise discretion and maintain confidentiality with regard to all company information
- Completion of all required trainings as designated by the company and accreditation/licensing entities.
- Other duties as assigned.
Licensure: Required:
- PMHNP or Nurse Practitioner license in New Jersey
- Minimum of 2 years’ experience in addiction medicine and/or family practice medicine
