Job Summary and Responsibilities
As our RN Care Coordinator, you will oversee the progression of care and discharge planning for identified patients, promoting quality and cost-effective outcomes while adhering to all hospital policies and regulations. Every day you will coordinate, communicate, and collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute providers to progress care toward optimal outcomes at the appropriate level. You will also advocate for patients and families, identifying and addressing barriers to care transitions, and striving to enhance the patient experience. To be successful in this role, you will be a Registered Nurse with strong clinical judgment, exceptional communication and collaboration skills, a deep understanding of care coordination principles, and a fervent commitment to patient advocacy and enhancing the overall patient experience.
- Discharge Assessment & Planning: Conduct and document comprehensive discharge assessments, updating plans as patient needs evolve to ensure clinical and psychosocial goals are met
- Multidisciplinary Coordination: Lead the development and implementation of discharge plans by collaborating with the healthcare team, patients, families, and post-acute providers
- Care Progression & Utilization: Partner with the clinical team to manage resource utilization and ensure the progression of care using evidence-based criteria
- Transition & Readmission Management: Facilitate seamless transitions to post-acute settings and implement targeted interventions to mitigate readmission risks for at least 30 days
- Patient Education & Advocacy: Serve as a patient advocate, ensuring all mandatory notifications are delivered, documentation is accurate, and patients are effectively educated on their care
- Compliance & Professional Standards: Maintain adherence to organizational mission, hospital policies, and regulatory standards while participating in performance improvement and ongoing professional education

Job Requirements
Required
- Graduate of an accredited school of nursing
- Minimum two (2) years of acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of 1 year experience
- RN license in the state(s) covered is required. North Dakota RN:ND or Compact
- BLS required within 3 months of hiring
Preferred
- Bachelor's Degree in Nursing (BSN) or related healthcare field
- At least five (5) years of nursing experience
- Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification

Our patients have access to 24/7 emergency care, swing-bed programs, and acute care services.
