What You'll Do
As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans designed to help residents improve mobility, restore function, and maximize independence.
Responsibilities include:
Perform comprehensive physical therapy evaluations.
Develop and implement individualized treatment plans based on resident needs and goals.
Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, and overall function.
Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.
Collaborate with physicians, nursing staff, therapy professionals, and the interdisciplinary care team to coordinate resident care.
Educate residents and families on therapeutic exercises, mobility techniques, safety strategies, and fall prevention.
Accurately document evaluations, treatments, progress notes, and discharge planning.
Participate in care planning and interdisciplinary discussions as appropriate.
Build positive relationships with residents while providing compassionate, patient-centered care.
Qualifications
Current California Physical Therapist (PT) license or eligibility for licensure.
Strong clinical, communication, and problem-solving skills.
Excellent interpersonal and organizational skills.
Passion for rehabilitation and resident-centered care.
Positive, collaborative, and compassionate approach to patient care.
