TITLE: Respite Skilled Nurse (LPN or RN) - Home Health Skilled Nursing

SERVICE AREA: Granger, IN

EMPLOYMENT TYPE: Long-term employment for Part-Time Weekday Evenings + Saturdays Schedule (25-30hrs/wk)

  • Will also consider Nurses seeking Full-Time hours (32-40/wk) so long as Part-Time care for current client can begin immediately, while additional client(s)/case hours are secured (approx. 15-45 days).

SCHEDULE: TYPE: 

  • Evenings x 3 days/wk for 6-7hrs/day (Tue, Thu, Fri from 4 or 5pm - 11pm), plus
  • Saturdays (x 1 day/wk) for 8hrs/day (12pm - 8pm)  
  • Family is flexible with dates and times per week.

We are seeking a highly skilled, compassionate, and professional Respite Skilled Nurse (LPN or RN) to join our home health agency in a Part-time, 3 Weekday Evenings + Saturdays (4 days total per week, approx. 25-30 hrs/wk) capacity to provide care and support for a lovely young adult female in Granger, Indiana.

About the Client:

  • This delightful, well-mannered female patient is in her mid-20s, has a wonderful family network around her, and requires assistance with ADLs, medication administration, among all standard areas of care.
  • Patient has occasional seizures, and is primarily non-verbal however does have some communication and acknowledgement capabilities.

Compensation (Weekly Pay):

  • RN Hourly: $37 - $40
  • LPN Hourly: $28 - $30

*Starting wage rates are based on experience, credentials, positive employment history & references, and overall fit.

 

Position Summary

Performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Reports to: RN Case Manager; Clinical Administrator/Director of Nursing

Qualifications

  • Graduate of an accredited school of professional nursing.
  • Current license to practice as a Licensed Practical Nurse or Registered Nurse in Indiana.
  • At least 1 year of paid, direct experience caring providing in-home care or similar is required (2+ years preferred).
  • Active CPR and First Aid Certifications required (or willingness to obtained prior to hire date through our agency or accredited third party).
  • Strong background in nursing, patient care, and confidentiality.
  • Effective written and verbal communication skills.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Ability and willingness to perform all duties and responsibilities as deemed necessary, including essential physical demands as outlined.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current driver's license or state ID, and reliable transportation.

Essential Functions/Areas of Accountability:

  • Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
  • Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
  • Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
  • Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
  • Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
  • Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
  • Recognizes and responds appropriately to potentially unsafe situations.
  • Demonstrates safe practice in the use of equipment.
  • Makes recommendations on improvement of safety.
  • Performs job in compliance with agency policies and procedures and professional and community standards.
  • Accepts responsibility for professional nursing role.
  • Follows applicable state and federal laws and agency standards.
  • Attends all meetings as required.
  • Maintains confidentiality in all aspects of the job.
  • Refrains from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
  • Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
  • Performs other related duties and responsibilities as deemed necessary.

Essential Physical Demands:

Frequent standing, walking/bending/stooping, occasional sitting

Weight handled; push/pull force: 10 - 20 lbs. occasionally, 20 - 30 lbs. infrequently; carry/lift: 5 -10 lbs. occasionally, 10 - 30 lbs. infrequently.
