As the Perinatal RN Navigator & Bereavement Coordinator at Community Regional Medical Center, one of California's largest birthing centers and one of only four hospitals in the state recognized by The Joint Commission as a Center of Excellence with Advanced Certification in Perinatal Care, you will support patients and families navigating complex perinatal diagnoses, pregnancy loss, fetal anomalies and other high-acuity maternal-fetal situations. In this unique role, you will lead the Perinatal Bereavement Program while serving as a trusted clinical resource, advocate, and partner throughout the patient care journey.

Responsibilities

You will coordinate individualized care plans that align with each patient's clinical, emotional and psychosocial needs — ensuring timely access to services and support across the continuum of care. Partnering with providers and interdisciplinary teams, you will facilitate communication, address barriers to treatment and help patients and families navigate complex healthcare decisions. You’ll connect families with community, financial and supportive resources and programs while promoting informed decision-making and continuity of care. In addition, you will contribute to program development through quality improvement initiatives, policy enhancement, staff education and mentorship in perinatal and bereavement care practices.

Qualifications

Education & Experience 

  • High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required 
  • 3 years of relevant experience in Labor & Delivery or Postpartum required 

Licenses and Certifications 

  • RN - Current and valid Registered Nurse license to work within the state of California required 
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required 
  • NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degree required
