  • Current, unrestricted RN license in state of residence and ideally compact states.

  • Wages: $68,000-$78,000 annually; geographical location impacts pay.

  • Monday - Friday

  • 8:00 am to 5:00 pm (subject to needs)

  • Six paid holidays

  • Must pass post-offer, pre-employment drug background tests as allowed by state, federal, local ordinance, statutes and licensing/accreditation requirements.

Job Title: Patient Care Nurse

Department: Operations

FLSA: Exempt

General Functions

Within the defined scope of nursing practice, assist pharmacists and staff members in reviewing and providing pharmaceutical care services to the patients of Amber Pharmacy and Hy-Vee Pharmacy Solutions. This includes but is not limited to oversight, monitoring, and proper documentation of patient medication and disease therapy management, as well as acting as a resource for subcontracted nursing services.

Primary Responsibilities:

  • Coordinates and aligns patient services with field resources as well as third-party vendors.

  • Evaluates the entire patient's history in making clinical decisions relating to quality healthcare decisions.

  • Analyzes and investigates concerns from patients, caregivers, providers, payers, internal staff, or others.

  • Handles incoming calls from internal and external customers as well as answering all nurse line and direct calls.

  • Calls the provider's offices for questions and clarifications. Establishes and maintains effective professional relationships with providers.

  • Enforces all state, federal, and local laws pertaining to nursing practice as well as Company policies and procedures.

  • Responsible for coordinating and delivering resources and information to optimize the patient and caregiver experience with specialty therapy with the ultimate goal of improving patient outcomes.

  • Coordinates the care of high-risk, post-transplantation and other specialty-targeted populations appropriately, efficiently, and cost-effectively.

  • Assists patients in arranging timely access to services and evaluates social/financial/environmental support adequacy in a culturally sensitive manner.

  • Utilizes known community resources to link patients with outside services/available programs. May participate in an interdisciplinary care team meeting to problem solve around highly complex care needs.

  • Interacts with patients via phone and in person to educate, motivate and empower them at every step of their care plan.

  • Documents patient adherence, side effect, and all information related to treatment and provide patient support as needed. Produce timely, accurate, and quality documentation.

  • Assists with injection training techniques and support.

  • Assists pharmacists in providing quality of life and annual clinical reassessment counseling to patients according to medication and/or disease state.

  • Works with healthcare providers to gather necessary medical documentation to support the coordination of treatment and ensure delivery of quality healthcare.

  • Consults with prescribers and patients regarding the use of medications and potential drug interactions, adverse reactions, and other clinical interventions.

  • Review and monitor patient care plan according to medication therapy and orders for safety and efficacy considering specific patient conditions and health status; reassess, update and review patient care plans and medications as appropriate.

  • Assists with the review of Prior Authorizations and Appeals requests.

  • Completes post-discharge assessments with patients.

  • Completes post-discharge assessments with patients.

  • Act as a central resource to internal and external staff for information on nursing and/or clinical issues.

  • Manage data entry and tracking to support a disease management approach for patients with chronic conditions.

  • Establish, coordinate and maintain a strong relationship with national organizations and state and local chapters, including Oncology Professionals, i.e., ACCC, ONS, MOC-ONS, Leukemia & Lymphoma Society, ASCO, NCCN, etc.

  • Develop and present informational/educational sessions to Amber Pharmacy staff.

  • Remain current with REMS programs, clinical requirements of conditions, and medications.

  • Keep abreast of industry trends and maintain knowledge of therapy by reading/reviewing literature, maintaining continuing education requirements, and attending in-services, seminars, conferences and other educational opportunities.

  • Represent the Company in a professional manner and appearance at all times.

  • Adhere to all company policies as indicated in the handbook and directives issued by management. Has reviewed Accreditation Policy and Procedure manual.

  • This is an office-based position.

Professional Competencies:

  • Proficient with MS Excel, Word, and Outlook

  • Self-initiative

  • Ability to work with all levels of internal management and staff, as well as outside clients and vendors

  • Ability to communicate effectively, both written and verbal, with all departments and at all levels of the organization

  • Excellent verbal and written communication skills and the ability to interact effectively with patients, co-workers, management, and other healthcare professionals

  • Display excellent judgment consistently

  • Ability to exercise discretion and maintain confidentiality to the level of required HIPAA standards

Required Qualifications:

  • Working knowledge of the healthcare industry

  • HIPAA trained and /or the ability to work with and protect highly confidential patient and employee information

  • Knowledge of state, federal, and Joint Commission guidelines/regulations/laws

  • Knowledge of pharmacology, infection control preferred

  • Knowledge of the technology necessary to administer high-touch therapies preferred

  • Standard nursing practices

  • Minimum of two (2) years of hospital/clinic or home healthcare environment, including patient and professional education skills preferred

  • Prior experience in treating and caring for patients with organ transplants, cancer, hepatitis, rheumatoid arthritis, multiple sclerosis, and other chronic conditions is highly desired

Educational Requirements:

  • Registered Nurse (RN) from an approved program by the National League of Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE)

  • A Bachelor of Science in Nursing (BSN) or Masters of Science in Nursing (MSN) is highly desired

Licensure and Certification Requirements:

  • Full, current, and unrestricted license as a Registered Nurse (RN) in Oncology or other specialized clinical certification is highly desired

  • Certified Case Manager Certification (CCM) or Accredited Case Manager (ACM) Certification is highly desired

  • Additional state licenses are highly desirable, but a willingness to obtain additional licensure will be required

  • Upon hire, obtain Company-supported advanced certification, education, and expertise 

Physical Requirements:

  • Must be able to remain in a stationary position up to 90% of the time

  • The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc., and traverse conferences, meetings, and remote events

  • Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer

  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations

  • The ability to observe details at close range (within a few feet of the observer)

Working Conditions:

Normal office environment.

Equipment Used to Perform the Job:

Computers, keyboards, mouse, monitors, fax, and/or headsets for phone work. Software specific to the position, including but not limited to Microsoft Outlook and Skype. Must lift and traverse the area to move paper and supplies to use the equipment.
