Your part in our UnitingCare team 

We are currently seeking to appoint a Part-time Case Worker to work as a part of the Clare Homes Supported Accommodation Program.

Our Case Workers support families experiencing homelessness with strengths-based, trauma-informed housing case management whilst they are in short-term accommodation.

The position will provide outreach and mobile support to these families.

As part of this role, you will:

• Deliver rapid and coordinated assistance to families in critical need of housing and/or support through triage, strengths-based assessment, collaborative case planning and trauma-informed crisis intervention.
• Apply a strengths-based, person-centred and trauma-informed approach in all interactions, recognising the strengths, resilience and goals of each family and promoting their safety, choice and self-determination.
• Respond to and action referrals identified during case management with families, advocating on behalf of families and linking them with specialist and mainstream services through warm referrals.
• Adhere to program guidelines regarding the appropriate use of allocated funds.
• Undertake administrative duties related to the operation of the program, including maintaining clear, concise and accurate case notes and records within the appropriate timeframes, in line with privacy, confidentiality and duty-of-care obligations.
• Identify, assess and inform line management of current or potential risks that may impact on service delivery and the safety of clients and staff, and work with families to develop safety plans where required.
• Participate in regular supervision and reflective practice to support safe, ethical and trauma-informed practice, professional development and worker wellbeing.
• Positively represent the organisation when engaging with all stakeholders.
• Undertake other duties as and when required within the scope and capabilities of the role.

What makes a difference for us?

• A tertiary qualification (Diploma minimum) in social work, human services, community services or related field is essential
• Knowledge of, and demonstrated ability to apply, strengths-based, person-centred and trauma-informed practice with people experiencing or at risk of homelessness.
• Lived experience of homelessness, trauma or navigating the service system is highly valued, along with the ability to draw on that experience to build trust and inform respectful, person-centred practice. Applicants are not required to disclose personal experiences.
• You must have excellent verbal communication skills that enable you to assess needs and strengths accurately and extensively as well as transferable written communication skills to record assessments on the database.
• Ability to build rapport and actively engage in a respectful, non-judgemental way with people who have a range of needs, including people who have experienced trauma, and to provide culturally safe and responsive support, respecting the cultures of all people you support and work alongside.
• Ability to work collaboratively as part of a team as well as responsibly within an autonomous setting, and to reflect on and develop your practice through supervision.
• Demonstrated competence with technology, particularly Microsoft Office and tools for data entry.
• Working with Children Check (Blue Card)
• Current Open Drivers Licence.
