Description
Make a meaningful difference in the lives of residents and their families!
At Oak Crest, we know that exceptional care goes beyond physical health. We're looking for a compassionate, organized, and people-focused Social Services Coordinator who is passionate about supporting older adults, advocating for their needs, and helping residents and families navigate life's transitions.
In this role, you'll be an important part of our interdisciplinary care team, building meaningful relationships with residents and their families while helping ensure every resident feels heard, respected, and supported.
What You'll Do
- Be a trusted advocate. Support residents' rights, preferences, and individual needs, ensuring their voices are heard in decisions about their care.
- Build meaningful connections. Get to know residents and their families, assess social and emotional needs, and provide ongoing guidance, counseling, and support.
- Make transitions easier. Coordinate admissions, discharges, internal transfers, and transitions to alternative care settings, helping residents and families navigate changes with confidence.
- Collaborate for exceptional care. Partner with nursing, therapy, and other team members to develop and coordinate individualized care plans that support residents' overall well-being.
- Connect people with resources. Serve as a liaison between residents, families, healthcare professionals, and community organizations to help residents access the services they need.
- Be there when it matters most. Provide emotional support, problem-solving assistance, and crisis intervention to residents and families facing challenging situations.
- Keep care on track. Complete required assessments, maintain accurate documentation, monitor care plans, and help ensure compliance with applicable regulations.
What We're Looking For
- Bachelor's degree in Social Work or a related field preferred but not required
- 1–2 years of experience in a healthcare or medical setting, preferably in long-term care.
- Strong communication, interpersonal, and organizational skills.
- A collaborative approach and the ability to work effectively with an interdisciplinary team.
- Compassion, empathy, and a genuine commitment to the dignity and well-being of older adults.
Why This Role Matters
You'll have the opportunity to make a real difference every day-whether you're helping a new resident feel at home, supporting a family through a difficult decision, or advocating for someone who needs a voice.
Apply today and help us make Oak Crest a place where residents feel valued, supported, and at home.
