- Associate's or Bachelor's degree in Nursing (ADN/BSN).
- Active New York RN license.
- MDS Assessor Certification (required).
- 1–2 years in long-term care with MDS coordination experience.
- Attention to detail, strong communication, and the ability to lead interdisciplinary teams.
- Manage the intake and evaluation process for patients in long-term care.
- Complete up-to-date patient assessments and oversee the interdisciplinary assessment for all nursing home residents.
- Monitor patient care by assessing procedures.
- Interview patients and record accurate medical codes.
- Enter medical codes and support medical billing processes, maintaining accurate medical records.
