We're seeking a dedicated and experienced Service Manager who share our commitment to our mission, vision, and values to join our team. The Service Manager will play a crucial role in overseeing and coordinating the delivery of support services to our clients, ensuring the highest standards of care and support are maintained.

Position Summary

The Admissions and Provider Recruitment Coordinator is responsible for recruiting host home providers, including reviewing applications, conducting interviews and home inspections, coordinating client matching meetings with the admissions manager, and partnering with HR to monitor background check and status updates. The position serves as the central operational link between host home providers, admissions, and administrative oversight. This position will also process and conduct client interviews across all service lines including employment services, daily and hourly SCL, IRSH and host homes.


Essential Duties and Responsibilities


Host Home Provider Recruitment

  • Conduct initial interviews with prospective Independent Contractors
  • Review and evaluate IC applications for completeness, eligibility, and compliance
  • Coordinate and complete home inspections to ensure service and regulatory standards are met
  • Maintain documentation related to IC onboarding and approval status
  • Complete IC reference checks
  • Schedule and attend introductions of IC to potential client matches
  • Track meeting between IC and potential client matches
  • Schedule and attend orientations once matches are approved


Matching Providers to Clients

  • Collaborate closely with Admissions Manager to match individuals seeking services with appropriate host homes
  • Participate in matching discussions and provide input on provider capacity, skills, and home environment
  • Support Host Home team in identifying emergency placements as needed


Essential Duties and Responsibilities

  • Conduct initial interviews with prospective host home providers and clients across all service lines
  • Coordinate and complete home inspections to ensure service and regulatory standards are met
  • Maintain documentation related to host home providers onboarding and approval status
  • Complete host home provider reference checks in collaboration with HR
  • Coordinate and attend matching/roommate meetings as needed.
  • Collaborate closely with the admissions manager and host home administrator to match individuals receiving services with appropriate host homes
  • Participate in matching discussions and provide input on provider capacity, skills, and home environment
  • Support Host Home team in identifying emergency placements as needed
  • Maintain up-to-date records of active Independent Contractors
  • Collaborate with HR to track and maintain an accurate list of active Relief Workers, including eligibility and training status
  • Support compliance audits and internal reviews related to IC contracts
  • Coordinate orientations with clients and their teams by scheduling, preparing paperwork and attending meetings

Requirements

Qualifications

Required:

  • Bachelor’s degree in human services, social work, healthcare administration, or related field (or equivalent experience)
  • Experience working with independent contractors, host home services, or HCBS services preferred
  • Strong organizational and documentation skills
  • Ability to conduct interviews, home assessments, and training sessions independently
  • Knowledge of confidentiality, compliance, and regulatory standards
  • Valid Iowa Driver’s License
  • Proof of 50/100/50K Automobile Insurance
  • Working cell phone
  • Ability to pass criminal history, motor vehicle, child abuse, dependent adult abuse and sex offender registry background checks
  • Possession of a motor vehicle that meets the required operational and maintenance guidelines, in accordance with the Iowa Department of Transportation motor vehicle safety standards and Candeo’s Motor Vehicle Guidelines


Preferred:

  • Experience in host home or community-based service models
  • Training or adult education facilitation experience
  • Familiarity with contract tracking and compliance systems
