Family Model Provider – Middle Tennessee

Position Overview

Mid-South Care Services is seeking compassionate, dependable individuals and families throughout Middle Tennessee interested in becoming Family Model Providers.

Family Model Providers welcome adults with intellectual, developmental, or physical disabilities into their homes, providing a safe, supportive environment while promoting independence, dignity, and quality of life.

Key Responsibilities

• Provide a safe, clean, and welcoming home environment.

• Assist with daily living activities, personal care, and individual needs.

• Promote independence, community involvement, and meaningful relationships.

• Follow each individual's Person-Centered Support Plan (PCSP).

• Provide transportation to appointments and community activities.

• Maintain accurate documentation and communicate with Program Coordinators.

• Ensure individual health, safety, rights, and regulatory compliance.

Qualifications & Requirements

• Must be at least 18 years old.

• Have a safe, stable home with appropriate accommodations.

• Maintain current Homeowners or Renters Insurance.

• Maintain current Automobile Insurance and reliable transportation.

• Complete all required Relias Training.

• Obtain and maintain Medication Administration Certification.

• Successfully complete required background checks and screenings.

• Complete required home inspections and ongoing monitoring.

• Maintain all required certifications and compliance documentation.

• Demonstrate compassion, dependability, and strong communication skills.

• Previous caregiving experience preferred but not required.
