Historian
It is our goal to provide the finest Orthopaedic care possible. This philosophy requires that all office staff and providers be sensitive and responsive to patients’ needs and preferences. To ensure that we hire and retain the quality of staff needed for implementing our philosophy of service, we have adopted the following job description for this position.
The position is an hourly position. Workdays are typically scheduled withing a five-day workweek. Hours and days may vary based on operational needs. All scheduling complies with applicable California wage-and-hour-laws.
Many of the responsibilities require that you have a working knowledge of computers and the ability to make proper use of the programs utilized in this office, or those that may be necessary to meet the needs of the practice.
BASIC FUNCTION:
Under the supervision of the Director of Workers and his/her designees, the Historian shall be responsible to complete their assigned reports and meet with physicians and management as needed.
JOB SUMMARY:
The WC Historian is responsible for interviewing patients and compiling a complete, comprehensive narrative on an examinee’s work-related injury, medical treatment history, and current complaints medico-legal reporting. The duties of the Historians are of high volume, and the quality of duties performed need to be at a high level. The Workers’ Comp Historian will come into contact on a daily basis with confidential patient files. They must be able to handle this information with the highest degree of privacy, discretion, and professionalism.
Duties and Responsibilities:
- Obtain comprehensive medical histories from injured workers for med-legal appointments.
- Thoroughly review all cover letters.
- Use appropriate template for provider and input all patient data in required fields. History must be addressed to all parties according to specification on cover letter.
- Create a shell for history and conduct a phone or in person history.
- Secure history prior to the appointment and document attempts in patient charts.
- Must include attestation paragraph with page numbers at the time of history.
- Responsible for securing and documenting interpreter information on the history (Name, Certificate, and agency)
- Verify facts cited on history and review medical records. (Mechanism of injury of each body part)
- Document treatment to date and outcomes and periods of disability. (Surgical Outcomes)
- Secure current complaints for all body parts and list the types of affected activities.
- Review past medical history for any prior injuries to address appointments according to 4663 and 4664.
- Document any prior injuries to specifi c body parts: auto accidents, work-related injuries, treatment, settlement and any residuals disability or ongoing symptoms.
- Maintain history schedule on time, working within the confines of the time allotted for history.
- Proofread history performed, checking for grammatical and spelling errors.
- Provide a visit prep to the physician outlining disputes and issues on the case.
- Email histories with med-legal paperwork to respective office (Patient History Packet Employee’s Disability Questionnaire) the day prior to the appointment to the respective office.
- Save history as word document in respective directory and transfer information into ECW.
- Work through assigned worklog for the month.
- Any other duties may be assigned by leadership.

Skill and Education Requirements:
- High school diploma or equivalence required.
- Associate degree or higher preferred.
- 1 to 2 years' work experience in Workers Compensation cases
- Experience in Orthopedics and Pain Management preferred
- Ability to type a minimum of 40 words per minute
- Proficient in operating a computer, scanner, and other offi ce equipment
- Working knowledge of Microsoft Outlook and other healthcare-related software applications.
- Strong telephone etiquette and customer service skills.
- Excellent verbal and written communication skills with patients, providers, and staff.
- Ability to multitask, prioritize responsibilities, and work effectively in a fast-paced, high-stress environment.
- Strong organizational and team coordination abilities.
- Ability to maintain patient confidentiality and comply with HIPAA regulations.
- Attention to detail and accuracy in documentation and recordkeeping.
- Demonstrated experience assisting providers with patient care, procedures, and clinical documentation.
- Bilingual is a plus.
- Must have experience with Med-Legal Worker's Compensation reporting
- Must have thorough understanding of AMC Guides 5th edition and Labor codes 4663 & 4664
Certificates, pre-employment requirements:
- Required to submit and obtain a negative result in the pre-employment drug screening.
- Ability to pass a background check.
