Job Title: Connected Living Partner 

Reports to: Director of Connected Living Experience

FLSA Status: Non-Exempt


Position Summary:

As a Connected Living Partner, you are responsible for creating an exceptional daily living experience for every resident. You provide compassionate, resident-centered support that promotes independence, dignity, safety, and meaningful connection. Connected Living Partners build trusted relationships with residents while supporting their physical, emotional, social, and wellness needs throughout the day.

This role combines caregiving, medication administration, dining support, life enrichment, hospitality, and light housekeeping into one seamless resident-centered experience. Connected Living Partners work collaboratively to ensure residents receive personalized support while living in a welcoming environment that feels like home.


Essential Duties and Responsibilities:

Resident Care & Wellness

  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, mobility, transferring, and dining assistance. 
  • Encourage independence while providing individualized support based on each resident's needs and preferences. 
  • Observe residents for changes in physical, emotional, or cognitive condition and promptly report concerns. 
  • Respond promptly to resident call systems and emergency situations. 
  • Assist with treatments and care activities as directed by licensed nursing staff. 
  • Protect resident dignity, privacy, and confidentiality at all times.

Medication Administration

  • Administer medications in accordance with physician orders, state regulations, and Provision Living policies. 
  • Follow the Rights of Medication Administration. 
  • Observe residents for medication effectiveness or adverse reactions and report concerns promptly. 
  • Accurately document medication administration and maintain medication records. 
  • Maintain medication security, storage, and inventory.

Dining & Hospitality

  • Create a welcoming dining experience by assisting with meal preparation, service, and cleanup. 
  • Escort residents to and from meals and provide dining assistance as needed. 
  • Deliver meals to resident apartments when appropriate. 
  • Encourage hydration and monitor nutritional intake as directed.

Engagement & Connection

  • Build meaningful relationships with residents through daily interactions. 
  • Encourage participation in activities, events, and wellness opportunities. 
  • Create meaningful moments throughout the day by engaging residents in conversation, activities, routines, and experiences that promote purpose, connection, and well-being.
  • Support families and visitors by creating a warm, welcoming environment. 

Community Support

  • Maintain a clean, safe, and welcoming home environment for residents through light housekeeping, laundry, organization, and attention to the details that contribute to resident comfort and quality of life.
  • Assist with laundry and other household responsibilities that contribute to resident comfort. 
  • Maintain supplies and equipment needed for daily operations. 
  • Collaborate with team members to ensure resident needs are met throughout the community. 

Team Collaboration

  • Communicate effectively with team members to ensure continuity of care. 
  • Participate in training and professional development. 
  • Support a culture of teamwork, accountability, and service excellence. 
  • Perform other duties as assigned.


Supervisory Responsibilities:

This position does not have any supervisory responsibilities.

Requirements

Job Requirements:  

Education

  • High school diploma or General Education Degree (GED) required. 
  • Successful completion of state-required caregiver and/or medication administration training, or the ability to obtain certification within established timeframes, as required by state regulations. 

Experience

  • Previous experience in senior living, healthcare, hospitality, customer service, or a related field preferred. 
  • Experience providing direct care and medication administration is preferred, where applicable. 
  • Demonstrated commitment to providing exceptional service and person-centered care.

Required Travel:

Travel is not required.


Physical Requirements:

This role requires regular physical activity in a senior living environment. Candidates must be able to:

  • Lift and/or move up to 50 pounds. 80 pounds of pressure may be placed on the associate through transfers using transfer belts or mechanical assists.
  • Stand, walk, and maintain a stationary position for extended periods.
  • Bend, squat, kneel, and move freely throughout the community.
  • Ascend and descend stairs or ramps as needed.
  • Position self to reach, operate, or adjust equipment above or below normal standing height.
  • Perform repetitive movements and tasks including grasping and fine manipulation.
  • Exposure to and ability to use cleaning chemicals and other equipment as part of routine duties.

Must be comfortable working in an environment that may involve exposure to infectious diseases and varying noise levels.
