Description
Position: Full-Time.
- Provide compassionate nursing care to hospice patients in their homes or care facilities.
- Perform routine nursing visits and assist with admissions as assigned.
- Monitor patient conditions and communicate changes to the RN Case Manager and physician.
- Administer medications and treatments as ordered.
- Educate patients and families on disease progression and comfort measures.
- Complete timely and accurate documentation in the EMR system.
- Assist with clinical coordination and daily office operations.
- Support scheduling, communication, and coordination with the interdisciplinary team.
- Upload and maintain documentation within the EMR system.
- Assist with physician orders, records, and clinical paperwork.
- Answer clinical phone calls and provide support to patients, families, and staff.
- Help maintain accurate patient records and regulatory documentation.
- Support the Executive Director and Director of Nursing with daily branch operations.
- Current Texas LVN license.
- Hospice, home health, or clinical nursing experience preferred.
- Strong organizational and multitasking skills.
- Excellent communication and customer service skills.
- Comfortable using electronic medical record (EMR) systems.
- Compassionate, dependable, and team-oriented.
- Valid Texas driver's license, reliable transportation, and current auto insurance.
- Current CPR certification.
