A few details about the role:
- Assist with simple health care routines such as reminders to maintain diet restriction, medication regimen, and recommended exercises.
- Provide observations and detailed feedback to the caseworker on resident’s behavior, mood, and adjustment in the home.
- Give emotional support and encouragement during period of loneliness, depression, bereavement, etc.
- Record observations and signs each entry on the appropriate form for each resident visit.
- Communicate on a weekly basis or more often as necessary with the nurse or director of home health.
- Immediately report emergency situations by phone to nurse or home health office.
- Confirm on a weekly basis the scheduling of visits with the scheduling coordinator, nurse, or director of home health to coordinate necessary visits with other personnel.
And here’s what you need to apply:
- High school diploma or equivalent.
- One year of experience in the home health setting preferred.
- Successful completion of at least eight hours of generic training in an approved basic education program.
- Current CPR certification.
- Must have a valid state driver’s license, a good driving record, and reliable transportation.
Compensation is based on experience and qualifications.
Pay is commensurate with experience.
Starting pay will be determined by skills, experience, and internal equity.
Pay determined by level of experience and position-related competencies.
