Overview
The Community Liaison serves as the primary relationship builder between PM Home Health/PM Residence and the surrounding healthcare community. You'll develop strategic partnerships, increase referrals, and support census growth. Key responsibilities include building relationships with physicians, case managers, discharge planners, and social workers; conducting outreach calls and visits; meeting with clients and families; maintaining sales records; representing the organization at community events; and collaborating with internal teams.
