What You Will Do

This is an exceptional opportunity for a motivated Primary Care physician to join a team within a busy, well-respected rural practice, supported by a strong referral base and the full resources of a robust health system network that is conveniently located on the campus of Ascension 

Calumet Hospital. 

 

• Join an established team of 1 physician and 5 APPs
• Call structure: 1:7
• Practice in a high-volume rural primary care clinic with a supportive, team-oriented environment within a busy and collaborative setting
• On campus with Calumet Hospital which was recognized as a 2024 Top 100 Critical Access Hospital by The Chartis Center for Rural Health for outstanding performance among the nation’s rural hospitals. 
• Employed position within a nonprofit health system
• Access to support staff
• Board Certified or Board Eligible]
• Eligible for state medical licensure and DEA registration
• Provide comprehensive patient care including evaluation, diagnosis, treatment, and appropriate documentation in compliance with payer requirements
• Collaborate on quality and cost improvement initiatives, with opportunities to participate in academic programs and inpatient/outpatient consultations, including call coverage
• Located in the scenic Chilton, WI area. Enjoy small-town charm with easy access to Appleton, Green Bay, and outdoor recreation

What You Will Need

Licensure / Certification / Registration:

• One or more of the following required:
  • Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
  • Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
• Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education:

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
