Hourly Pay Range:

$39.49 - $61.21 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

A Brief Overview:
The RN Case Manager is a pivotal member of the healthcare team responsible for optimizing patient care by coordinating and managing individualized care plans. This role involves assessing patient needs, collaborating with healthcare professionals, and ensuring seamless transitions across the healthcare continuum. The RN Case Manager is crucial in enhancing patient outcomes, minimizing healthcare costs, and ensuring a high standard of care delivery.

What you will do:

• Conduct comprehensive assessments of patients' medical, psychological, and social needs.
• Collaborate with physicians and interdisciplinary teams to develop personalized care plans.
• Identify potential barriers to care and proactively address them.
• Serve as the primary point of contact between patients, their families, healthcare providers, and community resources.
• Monitor and manage patient progress throughout their healthcare journey.
• Facilitate timely referrals to specialists, arrange for home healthcare services, and ensure seamless transitions during admissions and discharges.
• Educate patients and their families about their diagnoses, treatment options, and available support services.
• Promote patient self-management and empowerment to actively participate in their care decisions.
• Maintain accurate and up-to-date electronic health records, documenting all interactions, assessments, and interventions.
• Prepare reports and summaries to communicate patient progress and outcomes to the healthcare team.
• Collaborate with the interdisciplinary team to identify opportunities for improving care processes and patient outcomes.
• Participate in quality improvement initiatives and contribute to the development of best practices.
• Ensure compliance with all relevant healthcare regulations and standards, including HIPAA.

What you will need:

• Bachelors Degree Nursing Required Or
• Masters Degree Nursing Required
• 2 Years of clinical nursing experience, preferably in an acute care or hospital setting.
• Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. Required And
• Certified Case Manager (CCM®) - Commission for Case Manager Certification (CCMC) Certification in Case Management (CCM) or related field Required
