At Home With Van Dyk is seeking caring and compassionate Per Diem Certified Home Health Aides (CHHAs) to support hospice and home care clients throughout Bergen, Passaic, and Essex County. This is a great opportunity for CHHAs who drive, want flexibility, and are interested in completing multiple hospice cases in one day through cluster care opportunities.

What We’re Looking For

• Active NJ CHHA certification required
• Must be a driver with an insured, reliable vehicle and willing to travel to cases throughout Bergen and Passaic County
• Proficiency in spoken and written English required to perform job duties effectively
• Access to a computer or smartphone for web-based scheduling
• Must be caring, compassionate, dependable, and ready to support hospice clients

Must Have or Be Willing to Obtain/Provide

• NJ CHHA license/certification
• MMR vaccine documentation or titers
• Varicella vaccine documentation or titers
• Physical exam documentation
• Proof of COVID vaccine
• Flu vaccine documentation
• ID and Social Security card
• Must be open to drug testing
