Community Case Manager

Description

Eastern Area Agency on Aging is seeking a dedicated Community Case Manager (CCM) to provide ongoing, person-centered service coordination, information, and advocacy for older adults, people with disabilities, and their families. This a full-time, 32-hour-per-week, position plays a vital role in helping consumers navigate complex service systems so they can make independent, informed decisions about their care.

What You’ll Do

• Conduct Screenings & Assessments: Perform effective needs assessments by phone and in person to evaluate client needs, explain internal and external service options, and assist with completing benefits applications.
• Develop Person-Centered Care Plans: Create personalized care plans based on client preferences, needs, and program eligibility, keeping client involvement and self-sufficiency at the center of every decision.
• Provide Ongoing Coordination & Direct Support: Maintain regular contact with clients to monitor progress, adjust care plans, provide crisis support, and conduct home visits (upon supervisor approval) when client needs cannot be met remotely.
• Staff Access Centers: Provide in-person walk-in consultation and support at the Durgin Center in Brewer 2 days a week and at the Ellsworth Access Center once a month.
• Connect Consumers to Resources: Link clients to in-home and community-based services, non-publicly funded resources, and informal support networks beyond standard resource channels.
• Build Partnerships & Advocate: Communicate regularly and develop strong working relationships with family members, care providers, community service agencies, and regional organizations on behalf of consumers.
• Maintain Accurate Records: Record customer activity, client demographics, and referrals in the Wellsky database, ensuring documentation meets OADS requirements and is completed within 48 hours of client contact.
• Support Community Engagement: Participate in community outreach efforts, presentations, talks, and health fairs as needed, and utilize or direct volunteers to enhance client support.

Location & Schedule

• Work Schedule: 32 hours per week (occasional evening or weekend hours may be required for community events/presentations).
• Work Location: Hybrid model based out of EAAA's service area - 2 days per week in person at the Durgin Center in Brewer for walk-in client support, 1 day per month in person at the Ellsworth Access Center, and remaining hours worked are remote or field-based.
• Travel Requirements: Regular local travel to the Ellsworth Access Center monthly, the Durgin Center in Brewer, and local home visits as needed.

Requirements

What We’re Looking For

• Bachelor’s degree in Social Work, Human Services, Human Development, Gerontology, Health Services, or a related field.
• Prior experience in case management, service coordination, or related human services work is strongly preferred.
• Strong understanding of ethical social work boundaries and strict adherence to client confidentiality policies and procedures.
• Excellent verbal and written communication skills with the ability to build positive working relationships with clients, families, colleagues, and external agencies.
• Ability to work independently with minimal supervision, manage multiple competing priorities, and stay flexible in a changing environment.
• A proactive mindset for learning local/regional resources, finding innovative solutions for consumer needs, and operating standard or upgraded office equipment and database systems.
• Ability and willingness to complete required training for InformUSA CRS-A/D certification.
