Community Health Worker
Department: Primary Care
Employment Type: Full Time
Location: Columbia - Worley Street
Reporting To: Robert Gutierrez
Compensation: $20.98 / hour
Description
Schedule: Monday - Friday 8:00am-5:00pm.
Key Responsibilities
- Serve as a liaison between patients, providers, and community resources
- Assist patients with navigating healthcare, insurance, and social service systems
- Provide culturally appropriate education on health and wellness topics
- Support medication adherence, follow-up care, and self-management goals
- Help patients overcome barriers by arranging transportation, coordinating care, and advocating for needed services
- Document interactions and participate actively in the primary care team
- Build trust and promote health literacy within the community
- Support patients during post-hospitalization or after missed appointments
- Implement and document PRAPARE assessments as part of care planning
Requirements, Skills, Knowledge and Expertise
- Bachelor’s degree preferred
Successfully complete the accredited Community Health Worker program
- Believe in the power of community and the importance of accessible care
- Communicate with empathy and adapt well to different backgrounds and experiences
- Are highly organized, dependable, and comfortable working both independently and as part of a team
- Can navigate health and social systems—and help others do the same
- Are passionate about reducing barriers to care and promoting wellness across diverse populations
