Bereavement Counselor (Hospice) - part-time hoursJob Description
Bereavement Counselor provides grief assessment and support, as well as psycho-social services to patients and caregivers. Participates in programs that provide grief support and education for community members in the Hospice service area.
Hours: Part-time hours, flexible, daytime hours
Requirement: Bachelor's, BS or BA preferred
Responsibilities
Assists in the ongoing development of the bereavement program to improve care/services and their outcomes., Assists in training and supervision of Hospice volunteers., Assists with the implementation of all components of the Bereavement Program., Completes documentation accurately, thoroughly and timely per Hospice policy., Facilitates bereavement support groups and other educational programs for community members., Functions as a member of the Interdisciplinary team, attends Interdisciplinary team meetings to review and revise the plan of care and to report on the status of survivors., Maintains a caseload of survivors to provide bereavement follow-up consistent with policies of the Hospice program., Provides bereavement counseling, support and education to Hospice patients, caregivers, LTCFs, staff, and community members.Requirements
