Position Summary
We are seeking an experienced and detail-oriented MDS Nurse (RN) to oversee the resident assessment process and ensure compliance with all federal, state, and facility requirements. This role is responsible for coordinating and completing MDS assessments, supporting PDPM initiatives, monitoring quality outcomes, and collaborating with the interdisciplinary team to ensure accurate reimbursement and exceptional resident care.
Key Responsibilities
- Oversee and manage the facility's resident assessment process
- Ensure timely and accurate completion of all MDS assessments, including Medicare PPS, quarterly, annual, and significant change assessments
- Coordinate interdisciplinary participation in the MDS process and care planning
- Lead and participate in weekly PDPM meetings for skilled residents
- Monitor utilization and effectiveness of PDPM tools and processes
- Track and analyze Quality Measures (QM) and QAPI/QASP outcomes, providing recommendations for improvement
- Conduct assessments to determine resident health status and level of care upon admission
- Maintain the MDS schedule and ensure all assessment reference dates are met
- Complete data entry and electronic submission of MDS assessments
- Review, verify, and correct MDS submissions as necessary
- Coordinate and conduct resident care plan conferences
- Assist interdisciplinary team members in developing and revising individualized care plans
- Evaluate care plans for accuracy, effectiveness, and regulatory compliance
- Manage and communicate resident level-of-care changes throughout the facility
- Complete and submit required documentation to state agencies regarding level-of-care changes
- Provide MDS and PDPM education and training to nursing staff and new MDS personnel
- Maintain resident confidentiality and HIPAA compliance at all times
Qualifications
Education & Experience
- Current, unrestricted Registered Nurse (RN) license in South Carolina
- Graduate of an accredited School of Nursing
- Bachelor of Science in Nursing (BSN) preferred
- Long-term care experience preferred
- Minimum 3 years of supervisory experience preferred
- Strong knowledge of MDS, PDPM, Medicare reimbursement, care planning, and regulatory compliance
Skills & Abilities
- Experience with PointClickCare (PCC) preferred
- Proficiency with Microsoft Office applications
- Excellent analytical, organizational, and communication skills
- Ability to work collaboratively with interdisciplinary teams
- Strong understanding of quality measures and reimbursement systems
- Detail-oriented with a commitment to accuracy and compliance
Licenses & Certifications
- Current RN license in good standing
- Must maintain all required continuing education and licensure requirements
- Must remain in good standing with the South Carolina Board of Nursing
