ESSENTIAL RESPONSIBILITES:
- Provide nursing services to patients while ensuring a safe environment of care
- Always embody friendly and courteous customer service with both patients
- Act as an active participant in the healthcare team – working together with all levels of staff to ensure the best care possible for patients
- Work with team to meet specific healthcare performance improvement goals
- Adhere to all applicable HIPAA guidelines to ensure confidentiality of patient information
- Problem-solve issues as they arise in a pro-active and positive manner
- Organize and prioritize tasks as assigned
- Work with supervisor to evaluate data and make suggestions for improvement
Care Coordination:
- Prepare patients for scheduled telemedicine visits with their Primary Care Provider (PCP) by ensuring they are ready and available for the appointment.
- Assist patients with accessing and navigating telehealth platforms, including troubleshooting basic technical issues.
- Obtain and document vital signs, medication updates, health concerns, and other relevant clinical information prior to the telemedicine appointment.
- Ensure all necessary medical records, laboratory results, and patient information are available to the PCP before the visit.
- Facilitate communication between the patient and PCP during telehealth visits as needed.
- Reinforce the provider's care plan, patient education, and follow-up instructions after the telemedicine appointment.
- Coordinate referrals, testing, medication changes, and follow-up appointments resulting from the telehealth visit.
- Monitor patient adherence to treatment plans and communicate any concerns or changes in condition to the PCP.

Requirements
QUALIFICATIONS:
- Graduate of an accredited nursing program
- 1 – 2 years of experience preferred
- A current license from the State Board of Nursing
- CPR certified
- Excellent communication and problem-solving skills
- Ability to navigate electronic medical record systems
