Description

Be the Bridge to Stability. Transform Lives Through Housing.

Riverbend Community Mental Health—a trusted leader in New Hampshire’s behavioral health community—is seeking a compassionate, driven, and highly resourceful Outreach Counselor – Housing Bridge to play a critical role in helping individuals secure and sustain safe, stable, and meaningful housing.

This is more than a case management role—it’s an opportunity to be on the front lines of ending homelessness and advancing recovery for adults living with serious and persistent mental illness. At Riverbend, we recognize that housing is healthcare, and your work will directly impact an individual’s ability to heal, grow, and thrive.

As an Outreach Counselor, your work extends far beyond the office. You’ll be out in the community—building relationships with landlords and property managers, identifying housing opportunities, coordinating placements, and advocating for individuals every step of the way. From first application to long-term tenancy, you’ll walk alongside those you serve, providing hands-on support and encouragement as they transition into permanent housing.

You will be part of a mission-driven, collaborative team that is deeply committed to innovative, person-centered care. Your role will not only change individual lives—it will strengthen families, reduce hospitalizations, and contribute to a healthier, more equitable community.

If you are passionate about housing as a foundation for recovery, thrive in a dynamic, community-based environment, and are motivated to make a tangible difference every day, we invite you to bring your dedication and heart to Riverbend.

At Riverbend, you won’t just help people find housing—you’ll help them build stability, reclaim independence, and create a future filled with possibility.

Why Join Us?
Make a real impact—every single day. Join one of the state’s largest and most respected community mental health providers, where your work directly changes the lives of local individuals and families. At Riverbend, you’re not just filling a role—you’re helping transform lives and strengthen our community.

At Riverbend, you’ll be part of a collaborative, mission-driven team that is deeply committed to both client care and employee growth. We foster a supportive and inclusive environment where your voice matters, your contributions are recognized, and your career can thrive.

If you’re looking for meaningful work, a strong team culture, and real opportunities for professional growth through training, certifications, and advancement—Riverbend is the place to build your future while making a difference.
