Job Description

Davis Behavioral Health is actively recruiting a full-time Case Manager (SSW)/Team Lead to work on the Adult Mental Health outpatient team. This position will be based out of the DBH Layton Clinic office; however, the majority of the work will be conducted in the community throughout Davis County.

The Case Manager (SSW)/Team Lead is an experienced bachelor's-level mental health professional who serves as an operational leader within the Adult Mental Health Outpatient Case Management program. This position provides case management services while supporting team coordination, communication, workflow efficiency, and productivity. The Lead serves as a resource to case managers and peers, facilitates team meetings, assists with assignments and transfers, and promotes consistency in documentation, outreach, teamwork and service delivery.

As part of a multidisciplinary team, the Lead provides targeted case management, behavior management, skills development, and personal services to adults with serious mental illness and co-occurring substance use disorders. Services are delivered in office, home, and community settings with the goal of helping clients achieve and maintain independence and recovery.


Essential Job Functions

Primary job responsibilities assigned to the Adult Mental Health Outpatient Case Manager/Team Lead include but are not limited to the following:

  • Complete assessment at the required frequency to create individualized goals and objectives.
  • Use information from the assessment to complete individualized service plans in support of overall mental health treatment goals.
  • Maintain agency standards for productivity and documentation.
  • Engage with clients' treatment teams to support clients and coordinate treatment objectives.
  • Maintain an active caseload.
  • Coordinate and advocate for care that ensures basic needs are met. This may require coordination and advocacy with multiple local and State agencies.
  • Work with clients and their families or other support systems to ensure basic needs are met.
  • Be knowledgeable about the crisis system and know when to activate it.
  • Able to balance independent work while appropriately consulting with supervisor and taking directions when given.
  • Other duties and responsibilities as needed and assigned.

Position-specific Job Functions

In addition to the essential job functions outline above, the following responsibilities are assigned to your specific position:

Lead Responsibilities

  • Serve as a lead and trainer for Adult Mental Health Case Managers and Peer Support Specialists.
  • Promote consistency in case management practices, documentation, and workflow.
  • Support problem-solving and communication within the team and client complaints.
  • Facilitate one monthly Adult Mental Health Case Manager meeting.
  • Facilitate bi-monthly Peer Support meeting.
  • Participate in weekly Case Management assignment meetings with to:
  • Review referrals and assignments.
    • Coordinate transfers between staff as needed.
    • Review workload distribution.
    • Ensure assignment spreadsheets remain accurate and current.
  • Participate in biweekly peer assignment meetings:
    • Review peer referrals and assignments.
    • Coordinate transfers when appropriate.
    • Ensure outreach and assignment tracking is completed in team spreadsheets.
  • Coordinate team-building activities at least every six months to promote collaboration and engagement.
  • Other duties and responsibilities needed and assigned.

Employment Classification and Schedule

This position is classified as full-time, non-exempt, 40 hours per week.

The proposed schedule for this position is Monday through Friday from 8:00 am to 5:00 pm. Some flexibility is required to meet the needs of clients and to effectively meet with employers and community partners.

The Adult Mental Health Outpatient Team Lead/Case Manager is a full-time position with most hours Monday through Friday. DBH reserves the right to require occasional overtime, weekend work, and/or assignments to evening or night shifts.

Work hours must be during the regular business hours where the employee is available to meet with community employers and members of the treatment team.


Location & Work Environment

The office for the Adult Mental Health Case Management Outpatient program is located in the DBH Layton Clinic located at 2250 North 1700 West, Layton, Utah 84041.

The Adult Mental Health Outpatient Case Manager Lead will work in DBH offices as well as in the community, including clients' homes, DBH housing facilities, or other public locations. While this position will have office space within the agency, case management activities are expected to be performed across multiple settings including client homes and community locations. Significant fieldwork is required, including frequent travel throughout the county to conduct outreach, attend meetings, and provide direct client services. This position may require the use of a personal vehicle; mileage reimbursement is provided in accordance with organizational policies.


Compensation

The hourly rate for this position is outlined below:

BSW Degree + SSW License

  • $26.00 per hour base wage + 1% per year of licensure

Non-BSW Degree

  • $22.00 per hour base wage + 1% per year of direct social work experience, up to 10 years.

No Degree + BHCM Certification

  • $19.50 per hour + 1% per year of direct social work experience, up to 10 years.

Individuals who are bi-lingual English/Spanish are eligible for a $1,750 annual Spanish-language stipend available upon successfully scoring at least 80% on a language assessment.

All new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level, within the specified timeframes, and are a good fit for the job.

Team Lead Stipend

In addition to base wages, the Adult Mental Health Outpatient Case Manager/Team Lead will receive a $50.00 per bi-weekly pay period lead stipend. If the successful candidate leaves a lead role, the stipend will be discontinued.


Required Qualifications

The Adult Mental Health Outpatient Case Manager (SSW)/Team Lead must possess the following qualifications, skills, and abilities to successfully perform the essential functions of the job:

Experience

Required Experience

  • Demonstrated experience in the following areas is essential:
  • Ability to work diligently and maintain company standards without prompting.

  • Ability to develop and maintain effective working relationships with colleagues and clients.
  • Ability to communicate effectively orally and in writing.
  • Ability to motivate and engage clients.
  • General understanding of mental illness and substance use disorders.
  • Ability to maintain client confidentiality.

The ideal candidate will also have a working knowledge of mental health and substance use disorder services, community resources, experience working with clients that have severe mental illness, is hardworking, and can work independently as well as in a team environment.

Preferred Experience

  • 2 years experience working with individuals living with mental illness.
  • 2 years experience working in case management.

Education

  • Bachelor's degree in social work or a related field is required.

Licensure/Certification

Strong preference for active licensure as a Social Service Worker (SSW) in good standing in the State of Utah.

Candidates without an active SSW license may also be considered with the expectation that certification as a Behavioral Health Case Manager is completed within the first 30 days of employment (if not already obtained). Qualifications to become a certified Behavioral Health Case Manager through the Office of Substance Use and Mental Health include:

  • 400 hours of work, volunteer, and/or education experience in human services or a related field

Required 40-hour case management practicum and examination provided by DBH during the TCM certification class.

BHCM certification is not required for licensed individuals.

Skills/Abilities/Qualifications

  • Ability to meet or exceed productivity standards set by DBH.
  • Proficiently utilize required systems and/or equipment related to performing essential functions of the job.
  • Ability to independently manage schedule and assigned caseload, including scheduling appointments with clients, attending required meetings, coordination of care, supervision, and other work activities related to the completion of essential job functions.
  • Assist clients in developing a range of social support in the community.
  • Advocate for client rights.
  • Assist clients in monitoring their medications.
  • Skill in the application of problem-solving and multitasking.
  • Must be 18 at least 18 years old.
  • All employees at DBH must successfully pass a pre-employment criminal background check as well as a pre-employment drug screening. Since Davis Behavioral Health works with Federal grants, the company and its employees must comply with the Federal drug laws, including no use of medical marijuana.
  • Must have reliable transportation and a valid driver's license.

  • A clean driving record is also required to utilize the DBH vehicle fleet.

Preference will also be given to individuals who:

  • Have good organizational and practice management skills.
  • Can work independently and effectively.
  • Be flexible in days and hours available for scheduled work to best meet the needs of clients and DBH.

Bi-lingual English/Spanish is preferred.

The selected candidate must successfully pass a pre-employment criminal background check and drug screening. DBH will not hire individuals currently using illegal drugs or abusing alcohol; therefore, making employment at DBH conditional upon successfully passing pre-employment drug screening by providing a valid sample. Candidates who are offered employment are required to report for a drug screen at an Intermountain WorkMed location within 24 hours of being requested to do so by the DBH Human Resources Department.

Davis Behavioral Health works with Federal grants and must comply with the Federal drug laws, including no use of medical marijuana.

Those eligible must no longer be in active treatment at DBH and have one year of stable recovery. Active treatment includes weekly or group therapy, regular case management, housing, peer support services, etc. Clients who are in medication management services only or those who have infrequent therapy will be reviewed for eligibility on a case by case basis.

DBH utilizes an electronic medical health records system, so computer literacy and familiarity with Microsoft Office products is required.

Meeting monthly face-to-face service expectations and productivity standards is also required.

The successful candidate must also have a clean driving record to operate agency vehicles and/or transport clients, when necessary.
