Position Summary
The organization is seeking a Board-Certified or Board-Eligible Primary Care Physician to join its growing outpatient primary care team in Shelton, Connecticut.
This role includes:
- Comprehensive outpatient primary care across the lifespan
- Management of acute and chronic medical conditions
- Preventive care, wellness visits, and health screenings
- Chronic disease management including hypertension, diabetes, asthma, COPD, and other common conditions
- Coordination of care with specialists and hospital-based providers
- Collaboration with advanced practice providers and multidisciplinary support staff
- Opportunities to participate in academic, teaching, and quality improvement initiatives
Essential Position Responsibilities
- Provide comprehensive outpatient primary care services to pediatric, adult, and geriatric patients
- Diagnose and manage acute illnesses and chronic medical conditions
- Conduct annual wellness visits, preventive screenings, and routine physical examinations
- Develop individualized treatment plans and provide longitudinal patient care
- Order, review, and interpret laboratory and diagnostic studies
- Coordinate patient care with specialists and hospital-based teams
- Maintain timely and accurate documentation within the electronic medical record (EMR)
- Participate in shared phone-call coverage with minimal burden
- Collaborate effectively with physicians, APPs, nurses, and care coordinators
- Participate in quality improvement initiatives and organizational programs
Qualifications
Education
- M.D. or D.O. degree
- Completion of an accredited Family Medicine residency program
Licensure & Certification
- Board Certified or Board Eligible in Family Medicine
- Eligible for Connecticut State Medical Licensure
- Active or eligible DEA registration
- BLS certification required
Experience
- New residency graduates welcome
