Description

The Clinical Manager is a dynamic and experienced licensed Registered Nurse, Physical Therapist, or Occupational Therapist, who serves as the cornerstone of clinical excellence within the agency. This individual is responsible for the comprehensive planning, coordination, and management of all multidisciplinary home health services, ensuring that patient care is not only compliant and effective but also delivered with compassion and integrity. By providing direct oversight, mentorship, and expert clinical leadership to a dedicated team of field staff (RNs, LVNs, Therapists, etc.), the Clinical Manager champions a culture of quality, safety, and continuous improvement. This role is pivotal in translating the agency's strategic goals into day-to-day clinical practice, acting as the vital link between field clinicians, patients, physicians, and the agency's administrative leadership.

Key Responsibilities

1. Clinical Oversight and Care Coordination:

  • Directs and oversees the entire lifecycle of patient care, from admission to discharge, ensuring adherence to physician orders and established plans of care.

  • Conducts meticulous reviews and provides final approval for all clinical documentation, including OASIS data sets, plans of care (485s), and visit notes, to ensure accuracy, completeness, and regulatory compliance.

  • Leads interdisciplinary case conferences, fostering a collaborative environment where physicians, therapists, and nurses work together to create holistic and effective patient treatment plans.

  • Functions as the primary clinical expert and resource for field staff, providing real-time guidance on complex patient cases, challenging clinical scenarios, and evidence-based best practices.

  • Continuously monitors patient progress against established goals and outcomes, proactively ensuring that care plans are modified to reflect any changes in the patient's condition.

2. Staff Management and Development:

  • Provides direct, hands-on supervision, mentorship, and performance management for a team of field clinicians, fostering a culture of accountability and professional growth.

  • Conducts regular field supervisory visits (ride-alongs) to assess clinical competency, ensure adherence to agency policy, and provide individualized coaching.

  • Manages team productivity and performance metrics, addressing any disciplinary issues or performance gaps in a timely and constructive manner in collaboration with Human Resources.

  • Actively participates in the recruitment, screening, hiring, and comprehensive orientation of new clinical staff.

  • Identifies the educational needs of the clinical team and facilitates ongoing training, in-services, and professional development opportunities to keep the team current with the latest clinical skills and regulatory changes.

  • Cultivates a positive, collaborative, and supportive team environment that promotes clinical excellence, high morale, and staff retention.

3. Regulatory Compliance and Quality Assurance:

  • Guarantees that all clinical services are delivered in strict compliance with Medicare Conditions of Participation, state licensing laws, and Joint Commission standards.

  • Plays an active role in the agency's Quality Assurance and Performance Improvement (QAPI) program by collecting and analyzing clinical data, identifying trends, participating in chart audits, and implementing corrective action plans.

  • Serves as a key clinical leader in preparing for all regulatory surveys, audits, and responding to Additional Development Requests (ADRs).

  • Enforces agency standards to ensure all clinical documentation is accurate, timely, and defensible.

4. Operational Collaboration:

  • Works in close partnership with the agency Administrator and other non-clinical leaders to ensure seamless, efficient, and financially sound operations.

  • Collaborates with the intake and authorization departments to ensure the safe, appropriate, and timely admission of new patients.

  • Serves as a point of escalation for patient or family concerns, communicating with empathy and professionalism to ensure a high level of customer satisfaction.

  • Manages the strategic assignment of clinical staff to ensure appropriate caseloads, geographic efficiency, and skill-level matching for optimal patient care.

Qualifications

  • Education: Bachelor of Science in Nursing (BSN) from an accredited institution is strongly preferred.

  • Licensure: Must hold a current, unrestricted license as a Registered Nurse (RN) in the state of California.

  • Experience:

    • A minimum of three (3) to five (5) years of progressive clinical experience as a Registered Nurse.

    • A minimum of two (2) years of direct experience in a Medicare-certified home health setting is required.

    • Previous supervisory or management experience in a healthcare setting is strongly preferred.

    • Demonstrated experience with case management principles and practices.

  • Certifications:

    • Current CPR/BLS certification required.

    • Oasis certification (COS-C or HCS-O) is highly desirable, or willingness to obtain within one year of hire.

  • Clinical Expertise: Deep and current understanding of home health nursing practices, principles, evidence-based care, and complex disease management.

  • Leadership & Mentorship: Proven ability to lead, motivate, and develop a diverse team of clinical professionals, fostering a culture of accountability, respect, and continuous learning.

  • Regulatory Knowledge: Strong working knowledge of Medicare Conditions of Participation, state regulations, and accreditation standards. Must be able to interpret and apply complex regulations to daily practice.

  • Communication: Excellent verbal and written communication skills, with the ability to engage effectively and professionally with staff, patients, physicians, and families, including conflict resolution and de-escalation skills.

  • Critical Thinking: Superior critical thinking and problem-solving skills, with the ability to make sound, independent clinical and operational judgments in a fast-paced environment.

  • Technology Proficiency: High level of proficiency in the use of Electronic Health Record (EHR) systems for documentation review, reporting, and quality monitoring, as well as the Microsoft Office Suite.

  • Organizational Skills: Exceptional organizational and time management skills, with a proven ability to manage multiple competing priorities, delegate effectively, and meet deadlines.

  • Financial Acumen: Basic understanding of how clinical decisions, such as visit utilization and supply management, impact the financial health of the agency.
