Description
- Providing harm reduction advice in office settings (e.g. phone support) and outreach (home visits with staff, at other services)
- Provide support for clients with any unmet needs (e.g. health, housing, benefits, criminal justice)
- Supporting clients to access appointments e.g. attending with them to the hospital
- Supporting staff with home visits
- Provide suitable emotional support for clients
- Adhere to risk management, health and safety guidelines and any relevant policies relating to the role
- Help to complete comprehensive assessments & any other relevant paperwork, including writing up notes
- Potential to work up to recording information on SystmOne (our internal system) and co-working on a caseload
- Supporting staff with admin responsibilities such as phoning GPs, making pre-assessment phone calls with clients, phone check ins
- Attending team meetings where possible
