Seasonal Opportunity | Tentative Assignment End Date: June 2027

Compensation & Seasonal Incentive Opportunity

  • Starting pay: $17.80 per hour
  • Starting pay may be negotiable based on relevant experience, qualifications, and skills.
  • Daily Pay – Access your eligible earnings when you need them.
  • This is a seasonal position with a tentative assignment end date of June 2027, subject to business and client needs.
  • Employees who remain actively employed through the required duration of the assignment may be eligible for bonuses and/or other incentives.

Bonus and incentive programs, including eligibility requirements, performance expectations, payment amounts, and applicable terms, will be determined and communicated by Operations Leadership after joining Sagility.

What You Will Do

As a Care Coach, you will support adult patients throughout their healthcare journey. Many of the individuals you assist may be elderly, living with chronic conditions, or navigating complex healthcare needs.

Your responsibilities may include:

  • Conducting health assessments and gathering relevant patient information.
  • Helping patients understand available health benefits and resources.
  • Supporting care coordination and helping connect patients with appropriate services.
  • Communicating with members of the healthcare team to support continuity of care.
  • Educating and guiding patients as they navigate healthcare services.
  • Building trusted relationships with patients, healthcare professionals, and other stakeholders.
  • Applying critical-thinking and problem-solving skills to help address patient needs.
  • Accurately documenting interactions and maintaining information within electronic systems.

Qualifications

We welcome candidates who meet one of the following qualification paths:

Profile 1 – Allied or Social Science Background

  • Associate degree or higher in Allied Sciences, Social Sciences, Social Work, Human Services, or a related field.
  • Minimum of 1 year of direct adult patient care experience.

OR

Profile 2 – General Degree Background

  • Associate degree or higher in any discipline.
  • Minimum of 2 years of direct adult patient care experience.

Relevant patient care experience may include work in long-term care, home or community-based services, hospitals or other healthcare facilities, case management, managed care, or similar healthcare environments.

Nursing and physician degrees do not meet the educational profile requirements for this Care Coach position.

  • Strong verbal and written communication skills.
  • The ability to build rapport and trust with patients and healthcare stakeholders.
  • Compassion, patience, and professionalism when supporting individuals with complex needs.
  • Strong critical-thinking and problem-solving skills.
  • Attention to detail and the ability to accurately document patient interactions.
  • Proficiency with Microsoft Word, Excel, Outlook, and electronic record systems.
  • The ability to collaborate effectively as part of a healthcare team.

Preferred Experience

Preference may be given to candidates with experience in:

  • Supporting elderly and/or chronically ill populations.
  • Medicare and/or Medicaid populations.
  • Care coordination, case management, or managed care.
  • Conducting patient health assessments.
  • Chronic condition management and patient education.
  • Helping patients understand healthcare benefits and services.
  • Telephonic health coaching or remote care management.
  • Working as part of a multidisciplinary healthcare team.
  • Healthcare Business Process Outsourcing (BPO).
