Description

 The Hospice Certified Nursing Assistant (CNA) provides compassionate, hands-on care that supports the dignity, comfort, and quality of life of patients during end-of-life care. Working under the supervision of a registered nurse and as part of the interdisciplinary hospice team, the CNA delivers personal care and assistance with daily living activities, ensures the safe use of medical equipment, accurately documents services provided, and offers emotional support to patients and families. This role requires professionalism, empathy, and a strong commitment to patient-centered care in accordance with agency policies and regulatory standards.


This position is serving patients throughout the Aurora, Oswego, and Yorkville area.

  • Working in collaboration with the supervising RN, to fulfill the defined patient-specific Hospice CNA Plan of Care.
  • Collaborating with the care team as an extension of nursing services to ensure all patient needs are fully addressed.
  • Delivering hands-on personal care and supportive services to assist with safe activities of daily living, in accordance with the established Hospice CNA Plan of Care, and in compliance with the applicable laws, regulations, and agency policies.
  • Documenting care provided according to agency policy.
  • Using Durable Medical Equipment safely.
  • Participating in activities that promote patient safety, quality, and regulatory compliance.
  • Participating in professional development and lifelong learning activities.
  • Performing other responsibilities as assigned

Compensation & Benefits:

The pay range for this position is up to $23 per hour, depending on experience and qualifications. We offer a comprehensive benefits package, including:

Qualifications, Education and/or Experience: 

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required. 

  • Current Certified Nurse Aide (CNA) certification.
  • Current Basic Life Support/CPR certification.
  • Preferred hospice or home care experience.
  • Current and valid driver’s license, reliable transportation, and current liability auto insurance.
  • Ability to work well with others and take direction from management, as well as take initiative – willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
  • Excellent leadership, problem-solving, and communication skills. 
  • High attention to detail and accuracy.
  • Demonstrated ability to manage multiple tasks and deadlines in a fast-paced environment.
  • Self-motivated to accomplish identified goals with a strong sense of accountability for results.

Working Conditions:

  • The employee is occasionally required to stand/move, communicate, and identify written information. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to 50 pounds independently; occasionally lift and/or move up to 200 pounds with assistance.
  • This position may require working evenings, weekends, and holidays depending on patient needs. Flexibility in scheduling is required to accommodate on-call shifts or urgent patient care situations.
  • This role involves frequent travel to patients’ homes within the designated service area
  • Work is primarily performed in patients' homes, providing care in private residences, including living areas, bedrooms, and outdoor spaces, while ensuring comfort and dignity in a familiar environment
  • This role may involve emotionally challenging situations, such as providing end-of-life care and support for grieving families. The ability to manage personal emotions and maintain professionalism in these situations is essential.
  • While this role requires a high degree of independence, it also involves close collaboration with a multidisciplinary team including physicians, social workers, and chaplains. Regular communication and teamwork are essential to providing high-quality care.
  • This role requires active participation in staff activities that promote teamwork, unity, and morale. The individual will collaborate with colleagues to foster a supportive and cohesive work environment.
  • The individual must comply with all personnel policies, hospice care procedures, patient rights, and federal and state regulations, including HIPAA privacy laws and the scope of care provided in a hospice setting.
  • The individual will have access to patient medical records, ensuring confidentiality and HIPAA compliance while using this information to provide high-quality, personalized care in line with hospice care protocols
  • Employees are expected to follow all safety protocols, including infection control measures, to ensure a safe environment for patients, families, and staff. Personal protective equipment (PPE) may be required as per agency guidelines.
  • Participate in activities to promote patient safety, quality, and regulatory compliance.
  • Participate in professional development and lifelong learning activities.
  • Continually strives to improve nursing care skills through in-service education, formal education, workshops, conferences, active participation in professional organizations, and individual research or reading.
  • Individuals in this position must stay current on Dover Health training and ongoing education initiatives, actively pursuing self-improvement and continuous learning to enhance their skills in providing exceptional patient care.
